盐酸羟考酮注射液用于ICU机械通气患者纤维支气管镜检查的临床观察
Clinical observation of oxycodone hydrochloride injection used for fiberoptic bronchoscopy in ICU patients receiving mechanical ventilation
【摘要】 目的观察盐酸羟考酮注射液用于ICU机械通气患者纤维支气管镜检查的效果。方法 2014年4月~2015年4月,选取ICU机械通气患者90例,将其随机分为A、B、C组,每组30例,均进行纤维支气管镜检查,A组给予利多卡因气道表面麻醉,B组给予盐酸羟考酮静脉注射并利多卡因气道表面麻醉,C组给予丙泊酚静脉注射。观察3组患者在纤维支气管镜检查前、麻醉后、检查中的心率(HR)、呼吸频率(RR)、平均动脉压(MAP)及脉搏血氧饱和度(Sp O2)变化,记录检查中患者体动、呛咳情况以及检查后患者不良记忆率、愿意接受再次检查率。结果 3组患者检查中的HR、RR、MAP与检查前比较均升高(P<0.05),其中A组与B、C组比较升高最显著(P<0.05),而B、C组间比较差异无统计学意义(P>0.05);3组患者检查中的Sp O2与检查前比较均有所下降(P<0.05),但3组的Sp O2比较差异无统计学意义(P>0.05);A、B组实施麻醉后的HR、RR、MAP、Sp O2与检查前比较,差异无统计学意义(P>0.05),C组麻醉后的HR、RR、MAP与检查前比较有所下降(P<0.05);A组患者检查中体动、呛咳,检查后不良记忆发生率均高于B、C组(P<0.01),接受再次检查率低于B、C组(P<0.01),而B、C组体动、呛咳、不良记忆发生率,接受再次检查率比较差异无统计学意义(P>0.05)。结论 ICU机械通气患者行纤维支气管镜检查时应用盐酸羟考酮静脉注射并利多卡因气道表面麻醉简单易行,是一种安全有效的麻醉方式。
【Abstract】 Objective To observe the effect of oxycodone hydrochloride injection used for fiberoptic bronchoscopy in ICU patients receiving mechanical ventilation. Methods From April 2014 to April 2015,ninety patients in ICU receiving mechanical ventilation,and they were evenly and randomly divided into group A,B and C,and they were given fiberoptic bronchoscopy.Group A was given lidocaine for airway surface anesthesia,group B was given oxycodone hydrochloride for intravenous injection combined with lidocaine for airway surface anesthesia,and group C was given propofol for intravenous analgesia fibre bronchoscopy.The change of heart rate(HR),respiratory rate(RR),mean arterial pressure(MAP) and pulse oxygen saturation(Sp O2) before fiberoptic bronchoscopy, after anesthesia and during examination of patients in three group was observed respectively.body movement,bucking of patients during examination,and bad memory rate,the rate of the willing to re-examination of patients after examination was recorded respectively. Results Compared with before fiberoptic bronchoscopy,HR,RR,MAP of patients during examination in three groups was increased respectively(P<0.05),among them,the increasing group A was significant compared with group B and group C(P<0.05),but there was no statistical difference between group B and group C(P>0.05).Sp O2 was decreased in three groups during examination compared with before examination(P<0.05),but there was no statistical difference among three groups(P >0.05).HR,RR,MAP and Sp O2 in group A and group B after anaesthesia compared with before examination had no statistical difference(P>0.05),and HR,RR and MAP in group C was decreased respectively after anaesthesia compared with before examination(P<0.05).Compared with group B and group C,the incidence rate of body movement,bucking during examination,and bad memory after examination in patients of group A was higher respectively(P<0.01),and the rate of willing to re-examination in group A was lower(P<0.01).The incidence rate of body movement,bucking,bad memory and the rate of willing to re-examination had no statistical difference between group B and group C(P>0.05). Conclusion Oxycodone hydrochloride injection combined with lidocaine for surface anaesthesia used for patients of ICU receiving mechanical ventilation is simple for fiberoptic bronchoscopy,and it is a safe and effective anesthesia method.
