【摘要】 目的探讨羟乙基淀粉溶液联合地塞米松注入硬膜外腔治疗腰穿后低颅压头痛的临床效果。方法选取2013年2月~2015年1月本院神经内科收治的75例行腰穿脑脊液检查后发生低颅压头痛患者作为研究对象,随机分为空白组、对照组与观察组,各25例。空白组给予硬膜外填充生理盐水,对照组给予硬膜外填充低分子右旋糖酐联合地塞米松,观察组给予硬膜外填充6%羟乙基淀粉溶液联合地塞米松,比较3组的临床疗效。结果观察组的头痛发生率显著低于对照组和空白组,差异有统计学意义(P<0.05)。各组41~60岁年龄组的头痛发生率显著高于其他两个年龄段,差异有统计学意义(P<0.05)。观察组的VRS评分<4分所占比例显著高于空白组、对照组,4~6分所占比例和>6分所占比例显著低于空白组、对照组,差异有统计学意义(P<0.05)。结论羟乙基淀粉溶液联合地塞米松注入硬膜外腔治疗腰穿后低颅压头痛效果显著,值得临床推广应用。更多还原

【Abstract】 Objective To explore the clinical effect of hetastarch solution combined with dexamethasone injected into the epidural space in the treatment of intracranial hypotension headache after lumbar puncturing. Methods 75 cases with intracranial hypotension headache after lumbar puncturing from January 2013 to January 2015 in neurology department of our hospital were selected and randomly divided into the blank group,the control group and the observation group,25 patients in each group.The blank group was treated with epidural saline,the control group was given epidural filling low molecular dextran combined with dexamethasone,the observation group was given epidural filling 6% hydroxyethyl starch combined with dexamethasone.The clinical effect was compared among 3 groups. Results The incidence rate of headache in the observation group was lower than that in the blank group and the control group,with significant difference(P<0.05).The incidence rate of headache in age group from 41-60 among three group was higher than that in the other two groups,with significant difference(P<0.05).The proportion of VRS score less than four points in the observation group was higher than that in the blank group and the control group,the proportion of VRS score from 4 to 6points and more than 6 points in the observation group was lower than that in the blank group and the control group,with significant difference(P<0.05). Conclusion The effect of hetastarch solution combined with dexamethasone injected into the epidural space in the treatment of intracranial hypotension headache after lumbar puncturing is significant,it is worth of clinical promotion and application.更多还原
