【摘要】 目的探讨尿道内口漏斗形成对女性压力性尿失禁(SUI)的诊断价值。方法应用经会阴实时三维超声技术,观察90例女性SUI患者在静息及最大Valsalva两种状态下尿道内口漏斗有无形成,同时以239例正常女性作对照。以临床诊断作为诊断SUI的金标准,使用受试者工作(ROC)曲线评价尿道内口漏斗形成对SUI的诊断效能。结果静息状态下,SUI组及对照组均无尿道内口漏斗形成;最大Valsalva动作时SUI组的尿道内口漏斗形成数有50例,明显大于对照组25例,两组结果比较,差异有统计学意义。尿道内口漏斗形成诊断SUI的曲线下面积为0.725,其诊断SUI的灵敏度、特异度、阳性预测值、阴性预测值、准确度分别为55.6%、89.5%、66.7%、84.3%、80.2%。结论尿道内口漏斗形成对SUI有一定的诊断价值,可为临床提供诊断依据。更多还原

【Abstract】 Objective To investigate the diagnostic value of internal urethral orifice funnels formation in female stress urinary incontinence(SUI).Methods 90 cases of female stress urinary incontinence(SUI)were observed in two cases of the maximum Valsalva action and the resting state underwent real-time three-dimensional ultrasound,and239 cases of normal female were observed at the same time.Using clinical diagnosis as the gold standard for diagnosis of SUI,the diagnostic efficiency of SUI was evaluated by using receiver operating characteristic(ROC)curve.Results SUI group and the control group had no internal urethral orifice funnel formation in the resting state;the maximum Valsalva action,the number of internal urethral opening in SUI group was 50,significantly higher than the normal group.The difference was statistically significant between the two groups.The area under the SUI curve was 0.725.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of the diagnosis of SUI respectively were 55.6%,89.5%,66.7%,84.3%,80.2%.Conclusions The formation of interal ure funnel has a certain diagnostic value for SUI,which can be used as a basis for clinical diagnosis.更多还原
