子宫螺旋动脉血流参数与孕妇血清胎盘生长因子对早发型子痫前期预测
Study on the Combination of Blood Flow Parameter and Serum Placenta Growth Factor to Predict Early On-set Preeclampsia in Pregnancy
【摘要】 目的探讨正常妊娠与早发型子痫前期患者子宫螺旋动脉血流动力学参数及血清中胎盘生长因子(PLGF)水平的变化在预测早发型子痫前期中的价值。方法应用彩色多普勒超声对早发型子痫前期孕妇30例,健康孕妇58例,在未经任何药物治疗前,检测子宫螺旋动脉的血流参数。用酶联免疫吸附试验(ELISA)法测定血清胎盘生长因子(PLGF)含量。结果(1)子宫螺旋动脉血流动力学参数搏动指数(PI)、阻力指数(RI)、动脉收缩期最高血流速度与舒张期最低血流速度的比值(S/D)子痫前期重度组较轻度组、子痫前期组较对照组比值均增高,且差异具有统计学意义(P<0.05)。(2)血清胎盘生长因子(PLGF)值早发型子痫前期重度组与轻度组、早发型子痫前期组与对照组比较均降低,且具有显著性差异(P<0.05)。(3)子宫螺旋动脉参数PI与PLGF联合诊断早发型子痫前期的灵敏度为86.7%、特异度为86.2%。结论(1)彩色多普勒超声技术检测子宫螺旋动脉血流可以评估早发型子痫前期严重程度。其中,PI较RI、S/D能更好地预测子痫前期的发生。(2)孕妇血清PLGF联合PI预测早发型子痫前期的价值高于单个参数的使用。
【Abstract】 Objective To explore altered hemodynamics of uterine spiral artery and serum levels of placenta growth factor(PLGF)in normal pregnancy and patients with hypertensive disorder complicating pregnancy,and predict the value of preeclampsia.Methods Color Doppler ultrasound was used to detect hemodynamic of spiral arteries of uterus in thirty pregnant women with preeclampsia and fifty normal pregnant women before medical treatment.ELISA was used for detecting serum PLGF in all subjects.Results(1)The uterine spiral artery hemodynamics parameters PI,RI,S/D in preeclampsia group were higher than those in normal group(P<0.05).The hemodynamic parameters in pond-grade group were higher than those in low-grade group(P<0.05).(2)The contents of serum PLGF was lower in preeclampsia group than in normal group,and in severe preeclampsia group than in mild cases(P<0.05).(3)It is the sensitivity of 86.7%,86.2%that the combination of Uterine spiral artery PI parameters and serum PLGF diagnose preeclampsia.Conclusions(1)Color Doppler ultrasound detection of uterine spiral artery flow may be useful to predict preeclampsia during pregnancy and the value of PI is better than RI and S/D.(2)The value of the combination of serum PLGF and PI is better than that single parameter.
