血清胱抑素C检测在评估新生儿缺氧缺血性脑病中早期肾损害的意义及与其临床分度的关系
Clinical Significance of Serum Cystatin C in Early Renal Injury of Neonatal Hypoxic Ischemic Encephalopathy and the Relationship with Its Clinical Index
【摘要】 目的探讨血清胱抑素C(Cys C)检测在评估新生儿缺氧缺血性脑病(HIE)患儿肾功能及HIE程度中的临床应用。方法 HIE患儿163例,按照HIE分度,分为轻度组75例,中度组55例,重度组33例,三组均于出生后24 h内采集静脉血2 ml,进行血清Cys C、BUN和Cr检测,并将结果进行分析。结果 HIE患儿轻、中、重度三组血清Cys C分别为(1.46±0.27)mg/L、(1.63±0.25)mg/L和(1.83±0.49)mg/L,三组比较差异有统计学意义(P<0.01),轻度、中度和重度组三组组间比较差异均有统计学意义(P<0.05)。HIE患儿轻、中、重度三组血清血尿素氮(BUN)分别为(3.94±1.57)mg/L、(4.02±1.76)mg/L和(5.14±2.52)mg/L,三组比较差异有统计学意义(P<0.05),轻度与中度组比较差异无统计学意义;轻度与重度组,中度与重度组比较差异有统计学意义(P<0.05)。HIE患儿轻、中、重度三组血清Cr分别为(76.55±21.61)mg/L、(81.48±22.31)mg/L和(97.72±21.80)mg/L,三组比较差异有统计学意义(P<0.01),轻度与中度组比较无统计学意义;轻度与重度组,中度与重度组比较差异有统计学意义(P<0.05)。血清胱抑素C、BUN和Cr值与新生儿缺氧缺血性脑病严重程度均呈正相关(P<0.05)。结论血清Cys C改变可作为HIE患儿早期肾功能损害的评价指标,同时为判断HIE病情轻重提供依据。
【Abstract】 OBJECTIVE To investigate the serum Cystatin C(Cys C) in the assessment of renal function in neonatal hypoxic ischemic en-cephalopathy(HIE) and the application of HIE clinical index. METHODS Among 163 cases of HIE,75 cases were mild, 55 caseswere moderate,33 cases were severe,were collected venous blood 2ml in 24 hours. Cys C,BUN and Cr were detected,and the re-sults were analyzed. RESULTS Cys C in three groups were(1.46±0.27)mg/L,(1.63±0.25)mg/L and(1.83±0.49)mg/L, the differ-ence was statistically significant(P<0.01),and there were statistically significant differences between these three groups in mild,moderate and severe group(P<0.05). BUN were(3.94±1.57)mg/L,(4.02+1.76)mg/L and(5.14±2.52)mg/L, the difference wasstatistically significant(P<0.05),mild and moderate group was not statistically significant, mild and severe group,moderate andsevere group was statistically significant(P<0.05). Cr were(76.55±21.61)mg/L,(81.48±22.31)mg/L and(97.72±21.80)mg/L,the difference was statistically significant(P<0.01), mild and moderate group was not statistically significant, mild and severegroup,moderate and heavy degree group was statistically significant(P<0.05). The levels of Cys C,BUN,Cr were positively corre-lated with the severity of neonatal hypoxic ischemic encephalopathy(P<0.05). CONCLUSION The change of Cys C could be used asthe evaluation index of early renal function damage in HIE,and provide the basis for judging the severity of HIE.
