上海市浦东新区2013—2014年手足口病流行特征分析

【摘要】 目的探讨2013—2014年上海市浦东新区手足口病病原学特征及其流行趋势,为下一步加强防控工作提供有效依据。方法采集2013—2014年浦东新区临床诊断手足口病病例标本,运用实时荧光RT-PCR方法检测。结果 2013—2014年共检测手足口病样本823份,其中肠道病毒通用核酸检测阳性率为63.18%,EV71和Cox A16核酸检测阳性率分别为24.04%和30.19%,Cox A6和Cox A10阳性率分别为26.54%和1.54%,其他未分型肠道病毒占17.69%,2013—2014年的主要病原分别为Cox A6和Cox A16。1~12月份均有手足口病疫情报告,发病高峰集中在4~6月份。核酸阳性患者的男女性别比为1.50∶1,男性发病率显著高于女性,发病年龄集中在5岁以下,尤以3岁年龄组发病率最高,占阳性总数的34.62%。结论浦东新区2013—2014年手足口病流行的病原体主要为Cox A16,而Cox A6也已成为浦东地区手足口病流行的重要病原,今后应加强多种病原监测,做好防控措施是预防手足口病的关键。

【Abstract】 OBJECTIVE To investigate the pathogen characteristic and epidemiological trend of hand, foot and mouth disease(HFMD)of Shanghai Pudong New Area from 2013 to 2014, provide an effective basis for further prevention and control work. METHODS Wecollected the clinical diagnosis cases of HFMD of Pudong New Area from 2013 to 2014, and detected the HFMD virus by real-timeRT-PCR. RESULTS In all, 823 HFMD samples 2013 and 2014 were detected. The nucleic acid positive rate of common intestinal vi-rus was 63.18%, the positive rate of EV71, Cox A16, Cox A6 and Cox A10 were 24.04%, 30.19%, 26.54% and 1.54%, respectively,andother unclassified enteroviruses accounted for 17.69%. Therefore, Cox A6 and Cox A16 were the main pathogen in 2013 and 2014.There were epidemic situation report every month, and the peak incidence was concentrated from April to June. The sex ratio of nucle-ic acid positive patients was 1.50∶1, so the incidence of the male was significantly higher than the female. The age of onset was un-der 5-year-old at most, especially in the 3-year-old group, which accounted for 34.62% of the positive samples. CONCLUSION TheCox A16 was the main epidemic pathogen, and Cox A6 was also an important epidemic pathogen in Pudong New Area. In the future,strengthening the monitoring of various pathogens and take preventive measures would be the key for preventing HFMD.
