应用NO治疗新生儿呼吸重度衰竭疗效观察

【摘要】 目的探讨一氧化氮(NO)治疗新生儿呼吸重度衰竭疗效。方法选取140例新生儿呼吸重度衰竭患儿随机分为观察组和对照组各70例,对照组采用常规通气方法治疗,观察组在常规治疗的基础上采用吸入NO进行治疗,观察两组患儿治疗前、后的p H值、Pa O2、Sa O2及Pa CO2、临床效果、并发症、治疗时间及住院时间。结果两组患儿治疗后的Pa O2、Sa O2及Pa CO2比治疗前明显改善,而且观察组的改善程度明显优于对照组,差异有统计学意义(P<0.05)。观察组的总有效率(98.6%)明显高于对照组(80.0%),差异有统计学意义(P<0.05);通气治疗时间和住院时间明显短于对照组,并发症发生率(1.9%)明显少于对照组(14.3%),差异均有统计学意义(P<0.05)。结论采用NO治疗新生儿呼吸重度衰竭可以很好改善患儿的呼吸,减少治疗时间,并发症少,值得推广应用。

【Abstract】 Objective To explore the curative effect of nitric oxide( NO) in treatment of severe neonatal respiratory failure.Methods A total of 140 children with severe neonatal respiratory failure were selected and randomly divided into observation group and control group,70 children in each group. The children in control group were treated by conventional ventilation method,while the children in observation group were treated by NO inhalation therapy based on conventional ventilation method. p H values,Pa O2,Sa O2,Pa CO2,clinical effects,complications,treatment time,and hospitalization time before and after treatment in the two groups were observed. Results Pa O2,Sa O2,and Pa CO2 after treatment in the two groups were improved significantly compared with before treatment,especially in observation group( P < 0. 05). The total effective rate in observation group( 98. 6%) was statistically significantly higher than that in control group( 80. 0%)( P < 0. 05). The treatment time and hospitalization time in observation group were statistically significantly shorter than those in control group( P < 0. 05),the incidence rate of complications in observation group( 1. 9%) was statistically significantly lower than that in control group( 14. 3%)( P < 0. 05). Conclusion NO can improve breath and reduce treatment time with few complications in treatment of severe neonatal respiratory failure,which is worthy of popularization and application.
