北疆地区结核分枝杆菌基因分型及耐药性分析
Genotyping and drug resistance analysis of Mycobacterium tuberculosis in the northern regions of Xinjiang province
【摘要】 目的对北疆地区结核分枝杆菌临床分离株进行基因分型和耐药性检测,研究不同基因型在该地区的流行及耐药性。方法 收集北疆地区114株结核分枝杆菌临床分离株,采用间隔区寡核苷酸和多位点可变串联重复序列(VNTR)技术进行基因分型,应用NTsys 2.10e软件对结果进行聚类分析,比例法检测临床分离株的耐药性。结果 114株临床分离株,间隔区寡核苷酸分型结果和RD105缺失检测法结果共同显示北京基因型占62.28%(71/114)、非北京基因型占37.72%(43/114)。分2大簇(A和B):A簇有34株,北京基因型占11.77%(4/34),非北京基因型占88.23%(30/34);B簇有80株,北京基因型占83.75%(67/80),非北京基因型占16.25%(13/80)。耐药显示:总耐药率为40.35%(46/114),耐多药(MDR)率为19.30%(22/114);北京基因型耐药率为42.25%(30/71),非北京基因型的耐药率为37.21%(16/43),差异无统计学意义(x~2=0.28,P=0.60)。其中,北京基因型MDR耐药菌株占22.54%(16/71)结论 北疆地区北京基因型和非北京基因型的耐药率没有差异,但MDR菌株流行主要是由北京基因型引起的。
【Abstract】 Objective To study the relationship between different genotypes and drug-resistance in Mycobacterium tuberculosis and analyze molecular epidemiological characteristics.Methods One hundred and fourteen Mycobacterium tuberculosis clinical isolates were collected in the northern regions of Xinjiang province.Their genotypes were analyzed by spoligotyping and drug susceptibility was assessed using the proportion method.NTsys 2.10 e software was used to perform cluster analysis.Results There were 66 different genotypes among the 114 Mycobacterium tuberculosis isolates.Seventy-one(62.28%) isolates belonged to the Beijing genotype and 43(37.72%) to non-Beijing genotypes.The isolates divided into two clusters(A and B),with 34 in the A cluster,11.77%(4/34)of which were Beijing genotypes,88.23%(30/34) of which were non-Beijing genotypes;80 in the B cluster,of which 83.75%(67/80) were Beijing genotype isolates,and 16.25%(13/80) were Beijing genotypes.The drugresistance rate among the 114 Mycobacterium tuberculosis isolates was 40.35%(46/114),with 19.3%(22/114) of the isolates being multidrug resistance(MDR).The drug-resistance rate of Beijing genotype isolates was 42.25%(30/71),while that of non-Beijing genotype isolates was 37.21%(16/43)(x~2 =0.28,P=0.60).Of the drugresistant Beijing genotype isolates,22.54%(16/71) were multidrug resistant.Conclusion Mycobacterium tuberculosis isolates from the northern regions of Xinjiang province showed marked polymorphisms,with the main epidemic genotype being the Beijing genotype.Both Beijing and non-Beijing genotypes showed significantly high rates of drugresistance.
