Claudin-5、β-catenin和E-cadherin在胰腺实性-假乳头状瘤中的诊断价值

【摘要】 目的研究紧密连接蛋白Claudin-5、β-catenin和E-cadherin在鉴别胰腺实性假乳头状瘤(SPTP)与胰腺神经内分泌性肿瘤(P-NET)中的意义。方法收集2006-01—2014-06间60例SPTP、48例P-NET病例,回顾性复习临床资料、病理组织学特点,应用免疫组织化学检测NSE、Cg A、Syn、CD56、β-catenin、E-cadherin(ECD)和Claudin-5的表达情况。结果 NSE、Cg A、Syn、CD56免疫标记物在这2组肿瘤中都有不同程度的表达,P-NET组的Cg A阳性率显著高于SPTP组(97.9%:0),Claudin-5在所有SPTP中细胞膜(+),而P-NET均(-);β-catenin在60例SPTP呈细胞核(+),表达率为100%(60/60),在P-NET中呈细胞膜、细胞质(+),其表达率12.5%(6/48);ECD在60例SPTP中细胞膜(-)(0/60),在P-NET中呈细胞膜(+)(48/48,100%)。结论检测Claudin-5、β-catenin和ECD的蛋白表达有助于鉴别胰腺实性假乳头状瘤和神经内分泌肿瘤。

【Abstract】 Objective To investigate the expression of tight junction protein claudin-5,β-catenin and E-cadherin in solid pseudopapillary tumor of the pancreas( SPTP) and pancreatic neuroendocrine tumors( P-NET). Methods We retrospectively reviewed the clinical data,pathological features of 60 cases of SPTP and 48 cases of PNET between January2006 and June 2014. The expression of neuron-specific enolase( NSE),chromogranin A( Cg A),synaptophysin( Syn),CD56,β-catenin,E-cadherin( ECD) and claudin-5 was detected by immunohistochemistry. Results NSE,Cg A,Syn,CD56 expressed differently in SPTP and P-NET. The positive rate of Cg A in P-NET was higher than that in SPTP( 97. 9%VS. 0%). All cases of SPTP showed membranous expression of Claudin-5,whereas those of P-NET were negative; All SPTP cases( 60 /60) displayed nuclear expression for β-catenin,and none was positive for ECD. In contrast,12. 5%( 6 /48) of the cases of P-NET displayed membranous and cytoplasmic staining for β-catenin,and all P-NET cases were positive for ECD. Conclusion Detection of claudin-5,β-catenin and ECD is helpful to differentiate SPTP from P-NET.
