【摘要】 目的探讨干扰素α-2b治疗HBe Ag阳性慢性乙型肝炎患者早期病毒学应答的影响因素。方法选择2011年1月至2012年9月期间门诊及住院的HBe Ag阳性慢性乙型肝炎患者共53例,接受干扰素α-2b治疗至少24周。入组患者每4周随访1次直至24周,记录患者的一般临床资料、治疗基线时肝功能指标、HBV-DNA定量、乙肝两对半情况、血常规,以及每4周随访时的HBV-DNA定量、乙肝两对半情况等。采用Logistic回归模型分析干扰素α-2b初治HBe Ag阳性慢性乙型肝炎患者早期病毒学应答的影响因素。结果本研究中达到早期病毒学应答患者共29例,占54.72%;未达到早期病毒学应答患者24例,占45.38%。Logistic回归模型分析结果显示,基线病毒量(P=0.006)、患者抗病毒12周HBV-DNA定量下降量(P=0.001)、12周HBs Ag滴度下降量(P=0.001)是干扰素α-2b初治HBe Ag阳性慢性乙型肝炎早期病毒学应答的影响因素。结论干扰素α-2b治疗HBe Ag阳性慢性乙型肝炎患者早期病毒学应答的影响因素有基线病毒量、抗病毒12周HBV-DNA下降量和12周HBs Ag滴度下降量。更多还原

【Abstract】 Objective To analysis the influence factors of early virological response of interferon α-2b on the treatment of HBe Ag positive chronic hepatitis B patients. Methods A total of 53 HBe Ag positive chronic hepatitis B patients in the First Affiliated Hospital of Xiamen University from January 2011 to September 2012 were enrolled in the study retrospectively. All patients who weren’t treated with any antiviral drugs were assigned to receive 500 MU of interferon α-2b on alternate days. They were followed up every 4 weeks until 24 weeks. The clinical features, liver function, HBV DNA quantity,HBV immunological markers, blood routine examination at baseline and all follow-up times were recorded. The influence factors of early virological response of interferon α-2b were analyzed by the logistic regression. Results Early virological response occurred on 29 patients(54.72%). The result of the logistic regression showed that HBV DNA quantity at baseline(P=0.006), decreased HBV DNA quantity at the 12 th week(P=0.001), decreased HBs Ag quantity at the 12 th week(P=0.001) were predictors for early virological response of interferon α-2b for HBe Ag positive chronic hepatitis B patients. Conclusion The HBe Ag positive chronic hepatitis B patients who have lower HBV-DNA quantity at baseline,more decreased HBV-DNA quantity at the 12 th week, more decreased HBs Ag quantity at the 12 th week are more likely to have early virological response.更多还原
