神经生长因子制剂辅助血肿清除术对多灶性脑出血患者神经功能恢复的影响
Influence of hematoma aspiration assisted with nerve growth factor preparations for the recovery of neurological function in patients with multifocal cerebral hemorrhage
【摘要】 目的分析神经生长因子制剂辅助血肿清除术对多灶性脑出血患者神经功能恢复的影响。方法选取2013年8月~2015年8月延安大学附属医院神经外科收治的多灶性脑出血患者90例作为研究对象,按照随机数字表法将其分为观察组和对照组,各45例。对照组患者接受单纯血肿清除术治疗,观察组患者接受神经生长因子制剂辅助血肿清除术治疗。治疗15 d后,采用CT扫描并显示两组患者病灶区域血流参数,同时检测血清神经功能指标及炎症因子含量。结果治疗15 d后,观察组局部脑血容量、局部脑血流量高于对照组,平均通过时间短于对照组,差异有统计学意义(P<0.05);观察组脑源性神经营养因子含量高于对照组,神经元特异性烯醇化酶、髓鞘碱性蛋白、B型钠尿肽前体含量低于对照组,差异有统计学意义(P<0.05);观察组血清炎症因子白介素-6、白介素-10、白介素-11、肿瘤坏死因子-α、高迁移率族蛋白B1含量低于对照组,差异有统计学意义(P<0.05)。结论神经生长因子制剂辅助血肿清除术可增加多灶性脑出血患者的脑血流量,优化神经功能,具有积极的临床意义。
【Abstract】 Objective To analyze the influence of hematoma aspiration assisted with nerve growth factor preparations for the recovery of neurological function in patients with multifocal cerebral hemorrhage. Methods Ninety cases of patients with multifocal cerebral hemorrhage admitted to Department of Neurosurgery of Affiliated Hospital of Yan’an University from August 2013 to August 2015 were selected as research subjects, and they were divided into observation group and control group according to the random number table, each group had 45 cases. The control group received hematoma aspiration, the observation group received hematoma aspiration assisted with nerve growth factor preparations. After 15 d,CT scanning was used to test the blood flow parameters of lesions of the two groups, the serum nerve function indexes and the contents of inflammatory factors were detected. Results After treatment for 15 d, the regional cerebral blood flow, regional cerebral blood volume of observation group were higher than those of control group, the mean transit time was lower than that of control group, the differences were statistically significant(P < 0.05); the content of brain-derived neurotrophic factor of observation group was higher than that of control group, the contents of neuron-specific enolase,myelin basic protein, B-type natriuretic peptide precursor were lower than those of control group, the differences were statistically significant(P < 0.05); the contents of serum inflammatory factors interleukin-6, interleukin-10, interleukin-11,tumor necrosis factor-α, high mobility group protein B1 of observation group were lower than those of control group, the differences were statistically significant(P < 0.05). Conclusion Hematoma aspiration assisted with nerve growth factor preparations can increase the cerebral blood flow of patients with multifocal cerebral hemorrhage, optimize the neural function, which has a positive clinical significance.
