乌司他丁对急性肺损伤患者血清纤溶酶原激活物抑制剂-1、组织型纤溶酶原激活剂的影响
Effect of Ulinastatin on serum plasminogen activator inhibitor-1, tissue plasminogen activator in patients with acute lung injury
【摘要】 目的探讨乌司他丁对急性肺损伤患者血清纤溶酶原激活物抑制剂-1(PAI-1)、组织型纤溶酶原激活剂(t-PA)的影响。方法收集湖州市中心医院2014年9月2015年9月收治的急性肺损伤患者44例,将其随机分为对照组和实验组,每组各22例。对照组患者给予常规对症治疗,实验组在对照组的基础上给予乌司他丁治疗,治疗结束后,对所有患者的PAI-1、t-PA、抗凝血酶原Ⅲ(AT-Ⅲ)、动脉血氧分压(PO2)及氧合指数进行检测。结果治疗后,与对照组比较,实验组血清PAI-1、t-PA水平显著降低(P<0.05),血清AT-Ⅲ水平显著升高(P<0.05),动脉PO2及氧合指数显著上升(P<0.05)。结论乌司他丁能够显著降低急性肺损伤患者血清PAI-1、tPA水平,提高血清AT-Ⅲ水平、动脉血PO2及氧合指数,改善凝血纤溶系统功能,提高治疗效果,对临床有指导意义。
【Abstract】 Objective To analyze the effect of Ulinastatin on serum plasminogen activator inhibitor-1(PAI-1), tissue plasminogen activator(t-PA) in patients with acute lung injury. Methods Forty-four cases of patients with acute lung injury admitted to Huzhou Central Hospital from September 2014 to September 2015 were collected. All patients were randomly divided into control group and experimental group, with 22 cases in each group. The patients in the control group received conventional symptomatic treatment, the patients in the experimental group were given Ulinastatin treatment on the basis of control group. After treatment, the serum levels of PAI-1, t-PA, antiprothrombin Ⅲ(AT-Ⅲ), arterial blood oxygen pressure(PO2) and oxygenation index were detected in all patients. Results After treatment, compared with control group, the serum levels of PAI-1, t-PA in the experimental group were significantly decreased(P <0.05), the serum level of AT-Ⅲ was significantly increased(P < 0.05), and the serum levels of PO2 and oxygenation index were significantly rose(P < 0.05). Conclusion Ulinastatin can significantly reduce the serum levels of PAI-1 and t-PA, increase the serum level of AT-Ⅲ, the arterial blood PO2 and oxygenation index in patients with acute lung injury, and improve the function of coagulation fibrinolysis system and the therapeutic effect, which has a guiding significance for clinic.
