【摘要】 目的探讨曲安奈德联合白芍总苷胶囊治疗口腔扁平苔藓(OLP)的疗效及对血液流变学的影响。方法选取2014年2月至2015年1月青岛市口腔医院收治的100例OLP患者为研究对象,按照随机数字表法将患者分为联合治疗组和曲安奈德组,每组50例。曲安奈德组采用曲安奈德进行治疗(1mL2%的利多卡因与50g/L的曲安奈德注射液15mL配置成悬浊液,局部注射治疗,每周1次),联合治疗组在此基础上加服白芍总苷胶囊治疗(每次2粒,每日3次)。比较两组患者治疗前后临床症状、疼痛、血液流变学变化及治疗后的临床疗效与不良反应发生情况。结果治疗后,联合治疗组患者的半定量网纹-糜烂-溃疡评分和视觉模拟评分明显低于曲安奈德组[(7.1±1.3)分比(12.0±2.4)分,(1.7±0.5)分比(3.0±0.5)分],差异有统计学意义(P<0.01);联合治疗组患者的高切全血黏度、低切全血黏度、血浆黏度、红细胞沉降率显著低于曲安奈德组[(4.2±0.7)mPa·s比(5.4±1.0)mPa·s、(7.3±1.3)mPa·s比(9.5±2.0)mPa·s、(1.3±0.2)mPa·s比(1.9±0.2)mPa·s、(8.4±2.2)mm/1h比(10.7±2.6)mm/1h],差异有统计学意义(P<0.01);联合治疗组患者的总有效率明显高于曲安奈德组[92.0%(46/50)比74.0%(37/50)](P<0.05),疗效显著优于曲安奈德组(P<0.05)。联合治疗组和曲安奈德组患者的不良反应发生率比较差异无统计学意义[4.0%(2/50)比10.0%(5/50)](P>0.05)。结论曲安奈德联合白芍总苷治疗OLP能明显改善患者的临床症状、体征及血液流变学指标,缓解疼痛感,临床疗效较好,安全性高。更多还原

【Abstract】 Objective To study the effect of triamcinolone acetonide combined with total glucosides of paeony capsules on treatment of oral lichen planus( OLP) and its influence on blood rheology. Methods Total of 100 OLP patients in Qingdao Stomatological Hospital from Feb. 2014 to Jan. 2015 were collected,according to the random number table method,the patients were divided into a therapeutic alliance group and a triamcinolone acetonide group,50 cases in each group. The triamcinolone acetonide group was given triamcinolone acetonide( 1 mL 2% lidocaine and 15 mL 50 g / L triamcinolone acetonide configured suspended matter,local injection,once per week),the therapeutic alliance group was added total glucosides of paeony capsules( 2 pills per time,three times per day) on the basis of the triamcinolone acetonide group. The clinical symptoms,pain,blood rheology changes before and after treatment,and the clinical curative effect and adverse reactions of the two groups after treatment were also compared. Results After treatment,the REU and VAS of the therapeutic alliance group were obviously lower than the triamcinolone acetonide group [( 7. 1 ±1. 3) score vs( 12. 0 ± 2. 4) score,( 1. 7 ± 0. 5) scores vs( 3. 0 ± 0. 6) scores]( P < 0. 05). The total effectiveness of the therapeutic alliance group was significantly higher than the triamcinolone acetonide group[92. 0%( 46 /50) vs 74. 0%( 37 /50) ]( P < 0. 05),and the curative effect was better than the triamcinolone acetonide group( P < 0. 05). No obvious difference was found between the adverse reaction rate of the two groups[4. 0%( 2 /50) vs 10. 0%( 5 /50) ]( P > 0. 05). Conclusion Triamcinolone acetonide combined with total glucosides of paeony capsules could improve the OLP patient’s clinical symptoms,signs and blood rheology,relieve pain,with good clinical curative effect and high safety.更多还原
