三维超声观察伴盆底功能障碍的妊娠期女性盆膈裂孔结构变化的研究

【摘要】 目的应用三维超声观察伴盆底功能障碍的妊娠期女性盆膈裂孔的结构变化。方法将2015年3月至2016年3月于本院就诊的100例孕周20~30周的伴盆底功能障碍的妊娠女性纳入观察组,另将本院同期100例孕周20~30周的无盆底功能障碍的妊娠女性纳入对照组。所有研究对象均进行三维超声检查,比较两组研究对象静息期、缩肛期及张力期的盆膈裂孔前后径、左右径及面积。结果在静息期、缩肛期及张力期,观察组患者盆膈裂孔前后径、左右径及面积均显著大于对照组(P<0.05)。与静息期比较,两组研究对象的盆膈裂孔前后径、左右径及面积在缩肛期均减小(P<0.05),而在张力期均增大(P<0.05)。结论三维超声作为一种无创、快捷的检查手段,能够有效观察妊娠期女性盆膈裂孔的结构变化,对于临床评定妊娠女性发生盆底功能障碍的危险性具有重要意义。

【Abstract】 Objective To observe the structure changes of pelvic diaphragm hiatus in pregnant women with pelvic floor dysfunction by three-dimensional ultrasound. Method 100 cases of pregnant women with pelvic floor dysfunction, whose segestations were 20 to 30 weeks in our hospital from March 2015 to March 2016 were included in observation group. Another 100 cases of pregnant women without pelvic floor dysfunction, whose segestations were 20 to 30 weeks were included in control group. Three-dimensional ultrasound was performed in all the study subjects. The anteroposterior diameter, left and right diameter and area of pelvic diaphragm hiatus in the resting period, anal contraction period and tension period of the two groups were compared. Result In the resting period, anal contraction period and tension period, the anteroposterior diameter, left and right diameter and area of the pelvic diaphragm hiatus of observation group were significantly larger than control group(P < 0.05). Compared with the resting period, the anteroposterior diameter, left and right diameter and area of the pelvic diaphragm hiatus of the two groups had decreased in anal contraction period(P < 0.05), and had increased in tension period(P < 0.05). Conclusion As a non-invasive, fast examination method, threedimensional ultrasound can effectively observe the structure changes of the pelvic diaphragm hiatus of pregnant women. It can be used to evaluate the function of the pelvic cavity and prevent organ prolapsing, and is helpful to assess the risk of pelvic floor dysfunction in pregnant women.
