荆州市中心医院ICU感染患者病原菌分布及耐药性特点分析

【摘要】 目的 了解荆州市中心医院ICU住院患者感染病原菌的特点及其耐药情况,为临床合理使用抗菌药物提供依据。方法回顾性分析荆州市中心医院ICU2012年1月~2013年12月ICU住院患者标本中分离得到的病原菌资料,并对其耐药性情况进行分析。结果从所送检的810份标本中共检出568株病原菌,其中革兰阳性菌136株(占23.9%),以金黄色葡萄球菌、肺炎链球菌和屎肠球菌为主;革兰阴性菌389株(占68.5%),以大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌及鲍氏不动杆菌为主;念珠菌43株(占7.6%),均为白色假丝酵母菌。样本分布主要以呼吸道为主、尿路及血液感染次之。药敏结果显示,革兰阳性菌对万古霉素、利奈唑胺最为敏感,氯霉素次之。革兰阴性菌对亚胺培南最为敏感,哌拉西林,他唑巴坦次之。分离得到的病原菌对临床常用的抗生素耐药率均>50%。结论荆州市中心医院ICU患者感染的病原菌耐药情况严重,以革兰阴性菌为主,应定期监测病原菌的分布及耐药情况,为临床合理使用抗生素提供参考。

【Abstract】 Objective To investigate the distribution of the patients’ pathogens in ICU and their drug resistance in Jingzhou Central Hospital,and provide a basis for clinical rational drug use.Methods A retrospective analysis was conducted on the pathogen data of the patients admitted in between January 2012 and December 2013 as well as their drug resistance.Results 568 strains of pathogens were detected from 810 samples,includingl36(23.9%) strains of gram positive strains,389(68.5) strains of gram negative strains and 43(7.6%) Candida albicans.Gram positive strains were major in Staphylococcus aureus,Streptococcus pneumonia and Enterococcus faecium.Cram negative strains were major in Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa and Acinetobacter baumannii.Respiratory tract was the major model in sample distribution,followed by urinary tract and blood infection.The drug sensitivity results showed that gram positive strains were most sensitive to vancomycin and linezolid,followed by chloramphenicol,and gram negative strains were most sensitive to imipenem Pei lan,followed by Piperacillin/tazobactam.Resistance of pathogenic bacteria isolated were greater than 50%for antibiotic commonly used.Conclusion ICU pathogens,major in gram negative strains,have drug resistance in serious condition.They deserve an observation of the distribution and drug resistance for pathogenic bacteria.
