常规体检中健康人群心电图平板运动试验分析
Analysis of Treadmill Exercise Test in Subjects with Normal Routine Health Examination
【摘要】 目的观察分析单位常规体检者中,体检结果无异常,无心脏病史的志愿者的心电图平板运动试验的变化。方法随机选取466名作为研究对象。按照Bruce方案行亚极量平板运动试验,其中完成者465名。结果所有465名中结果阳性者34名(7.3%),可疑阳性28名(6.0%),阴性403名(86.7%)。其中男208名中结果阳性15名(7.2%),可疑阳性8名(3.8%),阴性185名(88.9%);女257名中结果阳性19名(7.4%),可疑阳性20名(7.8%),阴性218名(84.5%)。随年龄增加阳性率随之增高,男女性别比较,差异无统计学意义,不同年龄比较,差异有统计学意义(P<0.05)。结论在临床健康人群中,虽然常规体检没有异常发现,运动试验仍然能从中检出阳性、可疑阳性病例,提示其存在罹患心血管疾病的风险。
【Abstract】 Objective Electrocardiography exercise test is the most commonly used technique for detection stress- induced myocardial ischemia. Methods We selected 466 volunteers of men and women aged 22 to 70 to analyze the electrocardiographic responses of submaximal exercise with a multistage treadmil test in subjects with no history of heart disease or abnormal findings during routine health examination( ECG,Chest X- ray,Blood biochemical and Routine blood test). Results The number of participants who completed the treadmil examination according to the Bruce protocol was 465. 34( 7. 3%) had positive test results,28( 6. 0%) had suspected- positive test results,403( 86. 7%) had negative test results. In the men group,15( 7. 2%) of 208 were positive,8( 3. 8%) a suspected- positive results and 185( 88. 9%) a negative results. In the women group,19( 7. 4%) of 257 were positive,20( 7. 8%) a suspected- positive results and 218( 84. 5%) a negative results. Beyond 40 years old,25( 73. 5%) of 34 were positive and 22( 78. 6%) of 28 had suspected- positive results. There was statistical y significant difference among the age groups. There was no significance between women and men in the constituent ratio of results. Conclusion In the present study,although routine health- examination results were normal in all of the subjects,exercise test still found positive and suspected- positive results,implying possible myocardial ischemia risks in them. It is usefull to find cadiovascular disease in early stage.
