肺内磨玻璃结节影肺癌的早期诊断

【摘要】 目的探讨高分辨率CT(HRCT)和相关肿瘤指标在肺内磨玻璃结节影的诊断价值。方法 49例肺内磨玻璃结节影患者使用HRCT进行胸部扫描,同时检测血液中癌胚抗原(CEA)、糖类抗原125(CA125)、细胞角蛋白19片段(CY21-1)、神经元特异性烯醇化酶(NSE)水平,并结合病理结果进行ROC曲线分析。结果在肺内磨玻璃结节影直径≤3cm的病灶中,形态学上以磨玻璃结节影直径大以及含有实性成分比例高的肺内占位性病变的恶性可能性大。CEA、CA125、CY21-1和NSE判断恶性病灶的准确率分别为46.94%、59.18%、30.61%和26.53%,ROC曲线下面积分别为0.686、0.598、0.243和0.378。结论肺内磨玻璃结节影的HRCT形态学特征分析及相关肿瘤指标检测有助于肺癌的早期诊断。

【Abstract】 Objective To investigate the diagnostic value of high-resolution computerized tomography(HRCT)and tumor-associated indicators in pulmonary ground-glass nodules in pulmonary malignant lesions.Methods HRCT chest scanning was performed in 49 patients with pulmonary ground-glass nodules accepted with HRCT.Meanwhile,serum levels of carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),neuron specific enzyme(NSE)and cell keratin 19fragment(CY21-1)were detected.The receiver-operating characteristic(ROC)curve analysis of the tumorassociated indicators with the pathological results was carried out.Results In the cases with pulmonary ground-glass nodules≤3cm in diameter,the possibility of malignancy was increased in the pulmonary lesions with bigger size and solid component.The accuracy rates of CEA,CA125,CY21-1and NSE in the detection of malignant neoplasm were 46.94%,59.18%,30.61% and 26.53%,and the areas under ROC curve were 0.686,0.598,0.243 and 0.378,respectively.Conclusion The analysis of morphological characteristics of HRCT in pulmonary ground-glass nodules and detection of tumor-associated indicators are helpful to early diagnosis of lung cancer.
