【摘要】 目的观察经皮微创钢板内固定(MIPPO)技术结合肱骨近端锁定钢板(LPHP)治疗肱骨近端骨折的临床疗效。方法肱骨近端骨折患者53例分别采用MIPPO技术结合LPHP(A组,28例)和传统切开复位内固定(B组,25例)治疗。比较两组术中切口长度、手术时间、出血量、术后骨折愈合时间和肩关节功能恢复情况。结果 A组手术切口长度[(9.1±1.6)cm vs.(13.2±2.4)cm]、手术时间[(98.6±18.1)min vs.(118.3±8.6)min]和出血量[(122.2±21.9)ml vs.(310.2±64.6)ml]均少于B组(P<0.05)。随访6~20个月,两组骨折均一期愈合,无明显手术相关并发症。两组骨折愈合时间和肩关节功能恢复优良率(89.29%vs.84.00%)均无统计学差异(P>0.05)。结论与传统切开复位内固定比较,应用MIPPO技术结合LPHP内固定治疗肱骨近端骨折具有创伤小的优势。更多还原

【Abstract】 Objective To observe the clinical efficacy of minimally invasive percutaneous plate osteosynthesis(MIPPO)combined with locking proximal humerus plate(LPHP)in the treatment of patients with proximal humeral fractures.Methods Fifty-three patients with proximal humeral fractures were treated with MIPPO combined with LPHP(group A,28cases)and conventional open reduction and internal fixation(ORIF)(group B,25cases).Clinical efficacy of two groups was compared.Results Compared to group B,group A had shorter surgical incision[(9.1±1.6)cm vs.(13.2±2.4)cm]and operation time[(98.6±18.1)min vs.(118.3±8.6)min],and less blood loss during operation[(122.2±21.9)ml vs.(310.2±64.6)ml](P<0.05).The patients were followed up for 6to 20 months and the fractures were healed in the first phase without remarkable operationrelated complications.There were no significant differences in healing time for fractures and the excellent and good rate of functional recovery(89.29% vs.84.00%)between two groups(P>0.05).Conclusion Compared to conventional ORIF,MIPPO combined LPHP has the advantage of minimal invasion for the treatment of patients with proximal humeral fractures.更多还原
