【摘要】 目的 了解普外科腹腔镜阑尾切除术(LA)围手术期预防性应用抗菌药物情况，为临床合理使用抗菌药物提供参考。方法 采用回顾性方法对徐州医学院附属医院普外科2014年1月—12月114例LA患者进行调查分析，自行设计《LA围手术期抗菌药物应用情况调查表》,对LA患者围手术期抗菌药物临床预防应用情况进行调查统计与分析。结果 LA患者抗菌药物预防性使用率达100%,抗菌药物品种共涉及4类14种，抗菌药物使用频率前5位依次为甲硝唑57例次(22.09%),加替沙星36例次(13.96%),左氧氟沙星34例次(13.18%),头孢西丁31例次(12.02%),奥硝唑29例次(11.24%)。用药疗程为1～14天，平均用药时间(4.2±2.3)天，用药时间超过48 h者81例(71.05%);住院时间3～19天，平均(6±3)天。结论 徐州医学院附属医院LA患者围手术期预防性抗菌药物的应用基本合理，但仍存在一些不合理用药现象，需进一步加强临床抗菌药物应用的监管，提高临床合理用药水平。更多还原

【Abstract】 Objective To investigate the perioperative prophylactic use of antibacterials for patients with laparoscopic appendectomy and provide evidence for rational drug use. Methods Patients who underwent laparoscopic appendectomy(LA) in the Affiliated Hospital of Xuzhou Medical College from January 2014 to December 2014 were enrolled in the current study. Their clinical data were retrospectively analyzed. Then, a questionnaire about the perioperative prophylactic use of antibacterials for patients receiving LA was designed, in order to collect information for statistical analysis. Results The prophylactic use rate of antibacterial drugs was 100% in all patients. A total of 14 antibacterial agents were used. The top five antibacterial agent frequently used were metronidazole [57 cases(22.09%)], gatifloxacin [36 cases(13.96%)], levofloxacin [34 cases(13.18%)], cefoxitin [31 cases(12.02%)], and ornidazole [29 cases(11.24%)]. The duration of treatment was 1 to 14 days, with an average duration of(4.2±2.3) d. There were 81 patients who were treated with medication over 48 h(71.05%). The duration of hospital stay was 3 to 19 days, with an average of(6±3) d. Conclusion The perioperative prophylactic use of antibacterials for patients receiving LA is basically rational in the Affiliated Hospital of Xuzhou Medical College. However, irrational use of medications remains. Therefore, it is necessary to strengthen the control on the prophylactic use of antibacterial agengs, and to promote their rational use.更多还原
