肿胀手综合征的中心静脉CTA表现

【摘要】 目的分析总结189例肿胀手综合征患者的CT血管造影表现,探讨肿胀手综合征的中心静脉CTA特点。方法本组189例肿胀手综合征患者,男性84例,女性105例,年龄27~84岁,平均年龄(63.78±12.86)岁。本组病例均为维持性血液透析患者,血管透析通路建立时间5~184个月,平均(37.35±25.59)个月。CT血管造影图像经AW4.4工作站,应用最大密度投影、容积成像和曲面重建技术对图像数据进行处理和三维重建。结果 189例CT血管造影检查均获得了满意图像,血管狭窄病变73例,占38.6%(73/189),血管狭窄程度≥80%;血管闭塞病变116例,占61.4%(116/189)。104例在介入诊疗中,经数字减影血管造影(DSA)证实。结论肿胀手综合征是血液透析常见并发症,主要是因中心静脉狭窄或闭塞所致。CT血管造影可清晰显示中心静脉狭窄、闭塞的程度及部位,为临床治疗,特别是介入治疗提供准确的影像学信息,具有很高的临床应用价值。

【Abstract】 Objective To analyze and summarize the performance of CT angiography in 189 cases of patients with Swelling hand syndrome and to discuss the characteristics of central venous CTA with Swollen hand syndrome. Methods There were 84 male patients,105 female patients in this group of 189 cases of swollen hand syndrome,aged 27 ~ 84 years( mean age 63. 78 ±12. 86 years). Hemodialysis vascular access set-up time was 5 to 184 months( mean 37. 35 ± 25. 59 months). The patients were in maintenance hemodialysis patients,CT angiography images was taken as AW4. 4 workstation. We used the maximum intensity projection,volume rendering and curve econstruction techniques for image data processing and three-dimensional reconstruction.Results 189 cases of CT angiography images were obtained satisfactory,vascular stenosis was 73 cases,accounting for 38. 6%( 73 /189),vascular stenosis degree≥80%; vascular occlusion was 116 cases,accounting for 61. 4%( 116 /189). 104 cases in the interventional diagnosis and treatment were confirmed by digital subtraction angiography( DSA). Conclusion Swollen hand syndrome is a common complication of hemodialysis,mainly due to central venous stenosis or occlusion. CT angiography can clearly show the central venous stenosis or occlusion of the extent and location for clinical treatment,especially interventional radiological treatment,therehy providing accurate information,with a high clinical value.
