云南不同地区非小细胞肺癌EGFR基因突变检测分析

【摘要】 目的探讨云南不同地区非小细胞肺癌(NSCLC)患者的表皮因子生长受体(EGFR)基因突变情况。方法收集2012年5月~2015年1月本院云南不同地区NSCLC患者手术标本250例,其中昆明86例,宣威81例,其他地区83例。运用ARMS-Taqman探针法检测标本中EGFR基因突变情况。结果昆明地区86例中,48例(55.8%)存在EGFR基因突变,其中19Del、L858R占总突变的81.3%(39/48),有4例同时存在19Del、L858R突变;宣威地区81例中,43例(53.1%)存在EGFR基因的突变,其中19Del占总突变的14.0%(6/43),L858R占总突变的34.9%(15/43),G719X及S768I占总突变的41.9%(18/43),有4例同时存在G719X、S768I双突变;其他地区83例中,40例(48.2%)存在EGFR基因的突变,其中19Del、L858R占总突变的77.5%(31/40)。结论云南大部分地区EGFR基因突变情况与国内报道一致,主要以EGFR基因19Del及L858R突变为主;而宣威地区NSCLC患者中,EGFR基因突变主要以L858R为主,19Del的突变低于云南其他地区,S768I则明显高于云南其他地区。

【Abstract】 Objective To investigate the epidermal growth factor receptor(EGFR) gene mutation of patients with non-small cell lung cancer(NSCLC) from different regions of Yunnan. Methods 250 operative specimens of patients with NSCLC who were from different areas of Yunnan and hospitalized between May 2012 and January 2015 were collected, in which there were 86 cases from Kunming city, 81 ones from Xuanwei city, and 83 ones from other regions. ARMS-Taqman probe method was used to detect the EGFR gene mutation in those specimens. Results Among the 86 cases from Kunming, there were 48 ones(55.8%) of EGFR gene mutation,in which 19 Del or L858 R accounted for 81.3% of the total mutation(39/48). 19 Del and L858 R existed at the same time in 4 cases.Among the 81 cases from Xuanwei, there were 43 cases(53.1%) of EGFR gene mutation, in which 19 Del, L858 R, and G719 X or S768 I accounted for 14.0%(6/43), 34.9%(15/43), and 41.9%(18/43), respectively. Double mutation of G719 X and S768 I existed in 4cases. Among the 83 cases from other regions, there were 40 cases(48.2%) of EGFR gene mutation, in which 19 Del or L858 R accounted for 77.5%(31/40). Conclusion The situation of EGFR mutation in most regions of Yunnan is the same as that reported in our country, in which 19 Del and L858 R are the predominant gene. Among the NSCLC patients from Xuanwei, L858 R is the main gene mutation. Their proportion of 19 Del is lower than that of patients from other regions of Yunnan, but the proportion of S768 I is significantly higher than that of patients from other regions.
