儿童高血压与体质量指数腰围腰高比及腰臀比的关联性研究
Association between high blood pressure,body mass index,waist,waist-to-height ratio and waist-to-hip ratio among 7-17 years old children and adolescents
【摘要】 目的探讨儿童高血压与体质量指数(BMI)、腰围、腰高比、腰臀比的关联性,为儿童高血压和成年期高血压的早期预防提供参考。方法以2014年安徽省学生体质与健康调研检测中的8 890名7~17岁儿童(男童4 424名,女童4 466名)作为研究对象,测量其身高、体重、腰围、臀围、血压,计算体质量指数、腰高比、腰臀比,并进行Z分转化。分析高血压与各项肥胖相关指标Z分间的关系,采用相关性分析、多元线性回归模型、ROC曲线分析。结果 8 890名儿童的高血压检出率为13.61%(95%CI=12.90%~14.32%),其中男生(16.30%,95%CI=15.21%~17.39%)高于女生(10.95%95%CI=10.03%~11.87%),差异有统计学意义(χ~2=54.06,p<0.01)。BMI、腰围、腰高比、腰臀比4项人体测量指标Z分均呈正相关(P值均<0.01)。Logistic回归分析得出,4项人体测量指标均为高血压的影响因素(P值均<0.01),其中男、女童BMI的Z分指标对高血压的影响最大,OR值分别为1.79(95%CI=1.66~1.93)和1.69(95%CI=1.56~1.84)。ROC曲线结果显示,BNI、腰围的Z分指标与血压间的相关性较其他人体测量指标强。结论 BMI和腰围与7~17岁儿童高血压有良好的相关性。
【Abstract】 Objective To investigate relations between high blood pressure(BP),body mass index(BMI),waist,waist-toheight ratio(WHtR) and waist-to-hip ratio(WHR) among children and adolescents.Methods A total of 8 890 children and adolescents aged 7 to 17 years old(4 424 boys,4 464 girls) who participated in the Students’ Constitution and Health Survey in Anhui province.The height,weight,waist,hip-waist and BP were measured.The adiposity indicators,including BMI,WHtR and WHR,were converted into Z-scores before use.The relationship of high BP and adiposity indicators Z-scores were evaluated through Pearson correlation analysis,logistic regression model and receiver operating characteristic(ROC) curve.Results In this survey,the detection rate of high BP was 13.61%(95%CI= 12.90%-14.32%),and the rate in boys(16.30%,95%CI= 15.21%-17.39%) was significantly higher than that in girls(10.95%,95%CI= 10.03%-11.87%)(χ~2= 54.06,P<0.01).The indicators of BMI Z-scores,waist Z-scores,WHtR Z-scores and WHR Z-scores positively correlated with each other.Four body indicators were all risk factors of high BP by logistic regression analysis(P<0.01),the BMI Z-scores was the strongest risk factor for high BP in boys(OR= 3.1,95%CI= 2.12-4.53) and girls(OR = 3.07,95%CI= 1.76-5.34).The ROC showed that BMI and waist Z-scores were better for revealing high BP compared with other adiposity indicators.Conclusion Body mass index and waist Z-score are strongly associated with high BP among 7-17 year olds children and adolescents than WHtR and WHR.
