彩色多普勒超声对新生儿颅脑疾病的诊断价值
Clinical value of color Doppler ultrasound diagnosis on neonatal cerebral disease
【摘要】 目的探讨床旁彩色多普勒超声对新生儿颅脑疾病的临床诊断价值。方法应用床旁彩色多普勒超声对收治的有高危因素的284例新生儿(足月儿98例,早产儿186例,)的头颅进行检查,并将彩超检查结果与MRI进行对比分析,对超声检查出头颅疾病的大部分患儿进行10天~10个月超声随访检查,观察疾病转归情况。结果Ⅰ级颅内出血超声诊断64例,MRI诊断15例,超声诊断敏感性优于MRI,差异有统计学意义(P<0.05),Ⅱ级颅内出血超声诊断8例,MRI诊断8例,Ⅲ级颅内出血超声诊断1例,MRI诊断1例,Ⅳ级颅内出血超声诊断1例,MRI诊断1例,Ⅱ~Ⅳ级颅内出血、脑实质内出血、缺氧缺血性脑病、两者诊断敏感性相近,差异无统计学意义(P>0.05),室管膜下囊肿、脉络丛囊肿的超声诊断检出率明显高于MRI(P<0.05),蛛网膜或硬脑膜下腔出血MRI诊断敏感性优于超声,差异有统计学意义(P<0.05)。结论新生儿颅脑床旁多普勒超声对颅内出血I级、室管膜下囊肿、脉络丛囊肿的诊断优于MRI检查诊断方法,相比临床价值更高,可在临床推广应用。
【Abstract】 Objective To explore clinical diagnosis value of the bedside color Doppler flow imaging(CDFI)on neonatal cerebral disease.Methods 284 neonatal with high risk factors(186full terms infants,98 cases premature infants)were treated with color Doppler ultrasound.The results were compared with MRI.The children were followed-up for 10days-10 months.The disease outcome was observed.Results 64 cases diagnosed as grade I intracranial hemorrhage by CDFI and 15 cases by MRI.The sensitivity of CDFI was better than that of MRI(P<0.05).8cases diagnosed as grade II intracranial hemorrhage by CDFI and 8cases by MRI.The sensitivity of CDFI on gradeⅡ-Ⅳintracranial hemorrhage,parenchymal hemorrhage,intracranial hemorrhage hypoxia ischemic encephalopathy were similar to that of MRI(P>0.05).The sensitivity of CDFI on subependymal cyst and choroid plexus cyst were higher than that of MRI(P<0.05).The sensitivity of MRI on subarachnoid or subdural hemorrhage were better than that of CDFI(P<0.05).ConclusionNeonatal cerebral bedside Doppler ultrasound shows advantages compared with MRI.
