正颌外科二维头影测量与三维面颅重建测量的比较
Comparison of two-dimensional cephalometric and three-dimensional reconstruction facial-skull cephalometric in orthognathic
【摘要】 目的研究正颌外科手术患者二维头影测量与三维面颅重建测量的差异性,更精确的指导正颌外科手术设计。方法正颌外科手术患者术前及术后3个月均拍摄X线头颅定位侧位片和面颅CT三维重建,分别获得术前、手术模拟和术后的测量数据,并进行同项测量数据比较、分析。结果三维面颅重建测量与二维头影测量的部分同项测量数据(Go-Pg、N-ANS、ANS-Me)有统计学差异(P<0.05),术前二维模拟与术后二维头影测量部分值(GoPg、ANS-Me、ANB)同比有显著性差异(P<0.05),三维模拟与术后三维面颅重建测量值无统计学差异(P>0.05)。结论三维面颅重建可更真实、直观、立体地展示正颌外科手术患者的牙颌面畸形的细节和程度,并可进行三维数字化手术设计和模拟,三维重建测量结合传统二维头影测量可更精确的为牙颌面畸形患者进行诊断、手术设计和结果预测。
【Abstract】 Objective To research the difference of two-dimensional cephalometric and three-dimensional facial-skull reconstruction cephalometric among patients who had experienced orthognathic surgery,and then to precisely direct orthognathic design. Methods Patients took lateral skull X-ray and three-dimensional CT in pre-operation and three months after surgery.Then data were received,concluding pre-operation,post-operation and surgery simulation.The difference among data were compared and analyzed. Results Among two-dimensional and three-dimensional cephalometric,partial items(Go-Pg、N-ANS、ANS-Me)were significant differences(P<0.05), partial measurement(GoPg、ANS-Me、ANB) were significant differences between two-dimensional simulation and postoperative two-dimensional cephalometric data(P <0.05), no significant differences between tree-dimensional simulation and postoperative three-dimensional reconstruction data(P>0.05). Conclusion The extent and detail of dento-maxillofacial deformities were really and visually shown with three-dimensional skull reconstruction, we can design and simulate three-dimensional surgery.And we can provide precise diagnosis, surgery plan and results for dento-maxillofacial patients when we combine tree-dimensional and two-dimensional cephalometric.
