【摘要】 目的:对比研究军人Ⅱ型痛性足副舟骨的保守与手术治疗的疗效。方法:抽选2006-08~2012-06于我科诊疗的Ⅱ型痛性足副舟骨患者48例,随机分为保守与手术治疗两组,每组24例,分别给予保守及单纯副舟骨切除手术治疗,并分析治疗前后AOFAS中足功能评分,比较两组的治疗疗效。结果:48例患者中术后获得随访37例,手术组17例,保守组20例,平均随访时间13个月,手术组2例大运动量活动后仍有疼痛,保守组5例运动后疼痛,两组治疗后AOFAS中足功能评分均显著高于治疗前,手术治疗组疗效优于保守治疗组(P<0.01)。结论:系统的保守治疗和单纯副舟骨切除治疗II型痛性足副舟骨均可取得满意的疗效,但军人承担军事训练,保守治疗常难以系统进行,且病情易反复;单纯副舟骨切除手术对于军人Ⅱ型痛性足副舟骨有很好的治疗效果。更多还原

【Abstract】 Objective:To observe and compare the curative effect of conservative treatment and operation in soldiers with type II painful accessory navicular bone.Methods:Fourty-eight patients with type II painful accessory navicular bone diagnosed in our department from August 2006 to June 2012 were selected and divided randomly into conservative group and operation group(24cases in each group),which were given conservative treatment and simple excision of the accessory navicular bone respectively.The AOFAS scores were analyzed before and after the treatment to compare the curative effects of the two groups.Results:Thirty-seven patients were followed up with 17 cases in operation group and 20 cases in conservative group,and the averag follow-up time was 13 months.Pain remained after large amount of exercise in two cases of operation group and in five cases of conservative group.The AOFAS scores of all the patients were higher after operation than that before operation,and the efficacy was better in operation group than that in conservative group(P<0.01).Conclusion:Both of conservative treatment and simple excision of the accessory navicular bone can obtain satisfactory curative effects in soldiers with type II painful accessory navicular bone.But the conservative treatment is often difficult to be carried out and symptoms recurrence are easily to be found due to military training,while simple excision of the accessory navicular bone shows good curative effect.更多还原
