【Abstract】 Combined with root causes analysis,the failure mode and effect analysis( FMEA) was used to analyze all the failure modes and potential risks causes,risk priority number( RPN) was calculated and effective precautionary measures were formulated and implemented. After the intervention,the incidence of occupational exposure was significantly reduced from 42.86% to 7. 14%( P < 0. 05),and RPN was reduced from 1593. 94 to 611. 03.
