【摘要】 目的分析本院严重产后出血大量输血治疗的合理性。方法回顾分析入选的2011年7月~2016年7月本院严重产后出血24例临床资料。结果 24例患者出血量中位数为6 710 ml(3 880~20 480 ml),输红细胞量中位数为20 U(18~94 U),输注血浆量中位数为1 900 ml(800~9 400 ml)。13例患者输血小板,输注量中位数为8 U(8~80 U)。20例患者输注冷沉淀,输注量中位数为10 U(5~56 U)。20例患者输注纤维蛋白原,输注量中位数为6 U(4~22 U)。输注冰冻血浆:红细胞均数为(0.85±0.16)U,血小板:红细胞中位数为0.32(0~0.85),每输注10 U红细胞平均输注冷沉淀(4.37±4.46)U。结论本院严重产后出血大量输血治疗时,输注红细胞和冰冻血浆、血小板的比例较为合理,注重早期输注冷沉淀。更多还原

【Abstract】 Objective To study the current status of massive transfusion in life-threatening postpartum hemorrhage in our hospital. Methods A total of 24 cases of patients were retrospectively studied from July 2011 to July 2016. Results A total 24 cases of patients had an estimated postpartum hemorrhage of 6 710 ml(3 880 ~ 20 480 ml). The detailed transfusions were below :the average level of RBC was 20 U(18 ~ 94 U),the average volume of frozen plasma was 1 900 ml(800 ~ 9 400 ml). Platelet transfusion in 13 patients,median infusion dose was 8 U(8 ~ 80 U). 20 cases of cryoprecipitate,median infusion dose was 10 U(5 ~ 56 U). 20 cases of fibrinogen,median infusion dose was 6 U(4 ~ 22 U). The mean ratio of Frozen plasma to RBC transfusion was(0.85±0.16) U,the ratio of platelet transfusion to RBC transfusion was 0.32(0 ~ 0.85), while the ratio of Cryoprecipitate transfusion was(4.37±4.46)U of each 10 U RBC transfusiong.Conclusion The Frozen Plasma :RBC and Platelet :RBC ratio is rational in the patients with massive postpartum hemorrhage in our hospital,and early Cryoprecipitate transfusion was be emphasized in our massive transfusion protocol.更多还原
