【摘要】 [目的]了解肺癌患者住院费用的构成情况及其变化趋势,分析影响住院费用的主要因素,为降低肺癌患者住院费用提供参考依据。[方法]选取山西省某综合医院2003-2012年的肺癌住院患者为研究对象,采用方差分析和多元逐步回归分析对住院费用的影响因素进行分析。[结果]10年间该院住院治疗肺癌患者共计1441例,其中男性患者1028例(71.34%),女性患者413例(28.66%),期间平均住院天数呈增加趋势;住院费用由2003年的4600元(中位数)上升到2012年的13960元,年平均增长速度为7.66%。影响住院费用的主要因素依次是住院天数、手术与否和住院年份,其中住院天数对住院费用的影响最大,住院费用构成比药费超过45%。且住院时间越长,费用越高。[结论]肺癌患者的住院费用上涨速度过快,住院天数和手术与否是影响住院费用的主要因素,应加强对医院的管理,从缩短住院天数、规范合理用药、确立最佳治疗方案和探索合理的付费方式等几方面着手,降低肺癌患者的住院费用,减轻患者和社会疾病负担。更多还原

【Abstract】 Objectives To study composition status and variation trend of lung cancer patients hospitalization expense,analyzes main influence factors so as to provide references for reducing the hospitalization expenses of lung cancer patients. Methods It collected the lung cancer patients from 2003 to 2012 in a hospital,analyzed main influence factors of hospitalization expense by variance and multiple stepwise regression analysis. Results There were 1441 lung cancer patients in the hospital during 10 years which included male and female were 1028( 71. 34%) and413( 28. 66%). The average hospitalization days showed increase trend. The hospitalization expenses rose from 4600 Yuan( median) in2003 to 13960 Yuan in 2012,annual average growth rate was 7. 66%. Main influencing factors of hospitalization expense were hospitalized days,surgery or not and hospitalization particular year,hospitalization days was the maximum influencing factor to hospitalization expenses.Medicine expense was over than 45% in the hospitalization expenses. And the longer of hospitalization,the higher cost. Conclusions Facing rapidly increasing costs of lung cancer patients hospitalization costs,hospitalized days and surgery or not were main influence factors.
