股骨近端锁定钢板、动力髋螺钉及Gamma钉治疗老年股骨粗隆间骨折的临床疗效比较
Comparison of Proximal Femoral Iocking Plate, Dynamic hip Screw and Gamma nail in the Treatment of Intertrochanteric Fracture in the Elderly
【摘要】 目的探讨股骨近端锁定钢板、DHS及Gamma钉治疗老年股骨粗隆骨折的临床应用效果,为临床治疗老年股骨粗隆骨折选择手术内固定方式提供参考依据。方法随机抽取年龄在65-90岁股骨粗隆间骨折患者150例。依据不同的治疗方式,分为股骨近端锁定钢板治疗组、动力髋螺钉钢板固定(DHS)治疗组及Gamma钉治疗组,比较三组出血量、手术时间、骨折愈合时间、术后并发症以及髋关节创伤后功能评分等指标。结果在三组中Gamma钉治疗组出血量最少,手术时间最短,并发症少,早期Harris评分最高。结论 Gamma钉治疗组术中出血量明显较少、对患者损伤小,对各型骨折都能提供牢靠固定,术中术后并发症发生率低,骨折愈合时间短,是治疗股骨转子间骨折理想的手术方法。
【Abstract】 Objective To investigate the clinical effect of proximal femoral locking plate, DHS and Gamma nail in the treatment of femoral intertrochanteric fracture in elderly patients,in order to provide reference for the clinical treatment of elderly patients with femoral intertrochanteric fracture. Method 150 cases of intertrochanteric fracture in 65-90 years old were randomly selected.According to the different treatment, divided into proximal femoral locking plate treatment group, dynamic hip screw fixation(DHS) treatment group and Gamma nail treatment group.The bleeding volume, operation time, fracture healing time, postoperative complications and hip function score were compared between the three groups. Results In the three groups,the amount of bleeding of Gamma nail treatment group was minimal, and the operative time of Gamma nail treatment group was the shortest, and the complications of Gamma nail treatment group were minimal, and the early Harris score of Gamma nail treatment group was the highest.Conclusion Gamma nail in the treatment group, the amount of bleeding was significantly less, less damage to patients, can provide reliable fixation for all types of fractures,the incidence of postoperative complications was low,fracture healing time is short, is the treatment of intertrochanteric fracture of the ideal operation method.
