【摘要】 目的探讨子宫内膜癌中雌激素受体(ER)、孕激素受体(PR)、第10号染色体同源缺失性磷酸酶—张力蛋白基因(PTEN)、抑癌基因(p53)、细胞增殖核抗原(Ki-67)的表达与意义,以及术前血清癌抗原125(CA125)对子宫内膜癌的临床意义。方法收集100例子宫内膜癌患者临床资料、组织病理学结果、免疫组织化学结果、术前血清CA125结果,进行统计学分析。结果患者发病年龄越大、组织学类型为非内膜样内膜癌、手术—病理分期越晚、病理分级越高、肌层浸润越深,ER、PR阳性表达率越低(P﹤0.05);p53阳性表达率与患者发病年龄、绝经状态、病理分级有关(P﹤0.05);Ki-67阳性表达率与组织学类型、手术-病理分期、病理分级有关(P﹤0.05);血清CA125阳性表达率与组织学类型、手术-病理分期、病理分级、肌层侵润、淋巴结转移有关(P﹤0.05)。结论术前诊断性刮宫标本进行免疫组织化学检测ER、PR、PTEN、p53、Ki-67有助于明确子宫内膜癌的临床病理特征;术后子宫内膜癌组织标本免疫组织化学检测分子标志物联合检测血清CA125水平,有助于判断子宫内膜癌患者的预后,具有一定的临床意义。更多还原

【Abstract】 Objective: To investigate the expressions and significances of estrogen receptor( ER),progestogen receptor( PR),PTEN,P53,Ki-67 in endometrial carcinoma,and the clinical significance of the level of serum CA125 preoperative in endometrial carcinoma. Methods: The clinical data,histopathological rssults,immunohistochemical staining results and preoperative serum CA125 levels of 100 patients with endometrial cancer were collected and analyzed statistically. Results: The positive expression rates of ER and PR decreased with the increase of onset age,non- endometrioid carcinoma,late surgical-pathological staging,high pathological grading and increase of depth of myometrial invasion( P < 0. 05). The positive expression rate of p53 was correlated with onset age,menopausal ststus and pathological grade( P < 0. 05). The pasitive rate of Ki-67 was related to histological types,surgical- pathological staging and pathological grade( P < 0. 05). The positive rate of serum CA125 was correlated with histological types,surgical- pathological staging,pathlolgical grade,the depth of myometrial invasion and lympth node metastasis( P < 0. 05). Conclusion: Using the immunohistochenmical method to detect the expressions of ER,PR,PTEN,P53 and Ki-67 in specimens of diagnostic curettage in patients before surgery is helpful to make sure the clinical pathological characteristics of the endometrial cancer,and it with detecting preoperation serum CA125 level is helpful to predict the prognosis of patients,which has a certain singnificance in directing postoprerative treatment.更多还原
