降钙素原水平与感染源及病原学的相关性分析
Correlation analysis of procalcitonin levels with sources of infection and pathogenic microorganisms
【摘要】 目的探讨降钙素原(procalcitonin,PCT)水平与感染部位及致病微生物之间的相关性。方法采用回顾性研究方法,选择2012年8月—2014年8月入住SICU的脓毒血症患者(年龄20~85岁)120例,采集血标本检测PCT,并进行血培养、痰培养及尿培养确定致病微生物。结果无明确感染源的脓毒症患者PCT的平均水平3.79;肺炎患者PCT的平均水平24.87(P=0.038);尿路感染患者PCT的平均水平52.19(P=0.001);血源性感染患者PCT的平均水平27.73(P=0.005)。革兰阳性菌脓毒症患者PCT的平均水平25.87(P=0.016);革兰阴性菌脓毒症患者PCT的平均水平33.44(P=0.020);真菌感染脓毒症患者PCT的平均水平45.13(P=0.002)。结论不同的感染部位和致病微生物的PCT水平差异显著,PCT监测对不同感染部位及致病微生物具有一定的预测价值,可作为指导脓毒症患者诊断和治疗的重要指标。
【Abstract】 Objective To investigate the correlation of procalcitonin(PCT) levels with the infection sites and pathogenic microorganisms. Methods Retrospective analysis was done in 120 patients with sepsis,who were aged between 20 and 85 and admitted into Surgical Intensive Care Unit(SICU) between August 2012 and August 2014.Blood specimens were collected to measure PCT levels.Blood,sputum and urine samples were taken to determine the causative organisms. Results The mean PCT level of patients with unknown pathogen was 3.79,that of patients with pneumonia was 24.87(P=0.038),that of patients with urinary tract infection was 52.19(P=0.001),and that of patients with blood stream infection was 27.73(P=0.005).The mean PCT level of patients with sepsis caused by Gram-positive bacteria was 25.87(P=0.016),that of patients with sepsis caused by Gram-negative bacteria was 33.44(P=0.020),and that of patients with sepsis and fungal infection was 45.13(P=0.002). Conclusion PCT levels significantly vary among different infection sites and pathogenic microorganisms,which have definite predictive value and may play an important role in guiding the diagnosis and treatment of sepsis.
