非球面散光型人工晶状体治疗白内障合并规则角膜散光的临床分析

【摘要】 目的:观察非球面散光型人工晶状体(Acrysof IQ SN6ATT)在白内障合并规则角膜散光患者中的临床疗效。方法:选取在深圳市第二人民医院完成Acrysof IQ SN6ATT植入的规则角膜散光均>1.00 D的白内障患者12例(16只眼),术后随访3个月,观察术前、术后的总散光、裸眼视力(UCVA)、最佳矫正视力(BCVA)、角膜地形图、人工晶状体柱镜轴位。结果:术前患者的总散光、UCVA、BCVA平均分别为(-2.07±0.70)D、(0.86±0.39)Log MAR、(0.51±0.26)Log MAR,术后3个月患者的总散光、UCVA、BCVA平均分别为(-0.67±0.57)D、(0.26±0.24)Log MAR、(0.13±0.17)Log MAR,与术前相比,总散光减少,UVCA及BCVA明显提高,差异具有统计学意义(P<0.01);手术前后患者角膜散光无显著性变化;手术矫正散光向量(SIA)平均为(2.18±0.77)D,目标矫正散光向量(TIA)平均为(2.07±0.70)D,提示有轻度的过矫(CI=1.08±0.24);人工晶状体偏离预定轴位(4.94±3.05)°。结论:非球面散光型人工晶状体在矫正白内障患者术前规则角膜散光中,具有良好的安全性、有效性及可预测性,可更有效改善患者的视觉质量。

【Abstract】 Objective To observe the clinical results of implantion of the aspheric toric intraocular lens in cataract patients with regular corneal astigmatism. Methods 16 eyes of 12 patients with regular corneal astigmatism( > 1.00D) which underwent implantion of Acrysof aspheric toric intraocular lens(Acrysof IQ SN6ATT) were included.Patients were followed for total astigmatism,uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA),corneal topography and IOL cylinder axis within 3 months after surgery. Results The mean preoperative total astigmatism was(- 2.07 ± 0.70)D,and the mean UCVA and BCVA were 0.86 ± 0.39 and 0.51±0.26 Log MAR respectively.Postoperatively,the mean total astigmatism was(- 0.67 ± 0.57)D,and the mean UCVA and BCVA were 0.26 ± 0.24 and 0.13 ± 0.17 Log MAR respectively after 3 months.The differences were statistically significance(P < 0.001).There was no significant change in corneal astigmatism before and after surgery(t =- 0.734,P = 0.474;t=303,P=0.766).The mean magnitude of the SIA vector(2.18 ± 0.77)D was slightly higher than the mean magnitude of the TIA vector(2.07 ± 0.70)D at the last follow-up,and the mean correction index was 1.08 ± 0.24,all indicating minimal overcorrection.The mean absolute misalignment was 4.94±3.05 degrees. Conclusion Implantion of the aspheric toric intraocular lens proves to be a safe,effective and predictable method of correcting regular preoperative corneal astigmatism and provides better postoperative visual quality.
