急性脑梗死TOAST分型与ox-LDL和Lp-PLA2相关性的研究

【摘要】 目的:探讨血清氧化低密度脂蛋白(ox-LDL)和脂蛋白相关磷脂酶A2(Lp-PLA2)水平与急性脑梗死(ACI)患者TOAST病因分型及病情程度的关系。方法:选取2015年1月至2016年1月在深圳市第二人民医院神经内科住院治疗的急性缺血性卒中患者200例,进行TOAST分型(分成5个主要亚型,即大动脉粥样硬化型(LAA)、小动脉闭塞型(SAO)、心源性栓塞型(CE)、其他明确病因型(SOE)和不明原因型(SUE)及NHISS评分界定神经功能缺损程度分组(分为轻度神经功能缺损(<4分),中度神经功能缺损(4~15分),重度神经功能缺损(>15分),检测患者血清ox-LDL和Lp-PLA2水平,将结果进行统计学分析。结果:脑梗死TOAST分型中LAA及SAO组ox-LDL和Lp-PLA2水平高于对照组及其他亚型组,差异具有统计学意义(P<0.01)。脑梗死神经功能缺损中、重度组ox-LDL和Lp-PLA2水平明显高于轻度组,差异具有统计学意义(P<0.01);结论:联合应用ox-LDL及Lp-PLA2可以作为评判ACITOAST病因分型和疾病严重程度的重要和可靠的参考指标。
【Abstract】 Objective To explore the oxidized low-density lipoprotein(ox-LDL) and lipoprotein—associated phospholipase A2(Lp—PLA2) in Patients with acute cerebral infarction etiology plasma lipid levels and the relationship between TOAST classification and the degree of disease correlation analysis. Methods 200 patients with acute ischemic strokewere divided into five groups of LAA, SAO, CE, SOE and SUE group according to the TOAST classification. And Patients were divided into three groups of the light,medium and heavy defects group according to the neurological deficits with NHISS score. ox- LDL and Lp- PLA2 levels of all patients were determined by enzyme—linked immunosorbent assayl, the results for statistical analysis. Results 1. ox-LDL and Lp-PLA2 levels in the LAA and SAO group is higher than the control group and other groups(P < 0.01). 2. ox-LDL and Lp-PLA2 levels in the medium and heavy defects group is higher than the light group(P < 0.01). Conclusion Combined use of ox- LDL and Lp- PLA2 can be used as a TOAST etiology classification evaluation ofacute cerebral infarction and the importance of disease severity and reliable reference index.
