【摘要】 目的:探讨腹膜透析(PD)并发结核性腹膜炎(TBP)的临床表现及诊断方法。方法:对深圳市第二人民医院1例确诊为TBP的PD患者的临床资料进行分析讨论。结果:36岁女性,PD治疗3月余,反复午后发热20余天,腹部无明显压痛及反跳痛,多次查腹水常规白细胞均>100×10~6/L,普通细菌及真菌培养均阴性,常规给予抗感染治疗无效。进行腹水及血清淋巴细胞结核免疫3项检查后考虑为TBP,转往传染病医院给予抗结核治疗后好转。结论:TBP是PD患者少见的并发症之一。对于腹膜炎体征不明显,腹水常规提示白细胞>100×106/L,建议进行腹水及血清淋巴细胞结核免疫3项检查协助判断是否为TBP。更多还原

【Abstract】 Objective To realize the clinical manifestations and diagnostic method of peritoneal dialysis with tuberculous peritonitis. Methods There was one case who was diagnosed as peritoneal dialysis with tuberculous peritonitis,we reviewed her datum of cases and literature. Results This patient was 36-year-old woman,who accepted peritoneal dialysis therapy more than 3 months.She had a fever in the afternoon repeatedly,but she had no abdominal pain.The white blood cells of her peritoneal dialysis solution were all more than 100X10~6/L.Howeve,rcommon bacteria culture and fungus culture were all negative,conventional anti-infective therapy is invalid. She was diagnosed with tuberculous peritonitis after we examined the tuberculosis immune cytology of her serum and peritoneal dialysis solution.And then she was turned to the infectious disease hospital,she turned better after the antituberculosis therapy. Conclusion Tuberculous peritonitis is a rare complication in peritoneal dialysis patients.In order to defi ne the diagnosis, We should examine the tuberculosis immune cytology of serum and peritoneal dialysis solution for the peritoneal dialysis patients who had no abdominal pain and the white blood cells of peritoneal dialysis solution were more than 100×106/L.更多还原
