基于易损斑块生物标志物预警模型的构建
Early warning model for vulnerable plaque biomarker
【摘要】 目的:分析血清淀粉样蛋白A(SAA)、人胎盘生长因子(PLGF)、可溶性白细胞分化抗原40配体(s CD40L)、新喋呤(Npt)与急性冠脉综合征(ACS)易损斑块的相关性。方法:选择2012年12月至2013年12月住院疑似ACS患者90例,均行冠脉造影检查,术中据斑块形态分为稳定斑块(Ⅰ型斑块、Ⅲ型斑块),易损斑块(Ⅱ型斑块);所有患者术前常规测定血清SAA、PLGF、s CD40L、Npt及血脂水平,记录基线资料。利用Logistic回归分析方法筛选变量,探索易损斑块的主要影响因素。结果:SAA、PLGF、s CD40L、Npt均为发生易损斑块的危险因素,OR分别为1.61、1.88、1.96和1.79。结论:对胸痛患者进行SAA、PLGF、s CD40L及Npt生化标志物检测,对于预测冠脉易损斑块、指导临床治疗有重要意义。
【Abstract】 Objective To analyse the risk factors of vulnerable plaque biomarker and to construct an early warning system. Methods Ninety patients with suspected acute coronary syndrome(ACS) hospitalized during December 2012 and December 2013 were selected. The coronary artery lesions were divided into type I,II and III plaque groups by the morphology of atherosclerotic plaque. Serum SAA, PLGF, s CD40 L and Npt were measured. The results of SAA, PLGF, s CD40 L and Npt were compared. Logistic regression model was fitted to explore the main influencing factors of the vulnerable plaque. Results SAA, PLGF, s CD40 L, and Npt were main influencing factors of the vulnerable plaques, and the ORs were 1.61, 1.88, 1.96 and 1.79 respectively.Conclusion The detection of SAA, PLGF, s CD40 L and Npt biochemical markers in patients with chest pain is important for predicting the vulnerable plaque and guiding clinical treatment.
