【摘要】 目的探讨多层螺旋CT(MSCT)与数字摄影检查(DR)在四肢骨关节结核(BJTB)诊断中的应用价值及影响因素。方法收集我院经手术病理或穿刺病理学证实的四肢骨关节结核86例,全部病例确诊前均行MSCT与DR检查,所有MSCT图像[包括轴位、CT容积重组(VRT)、最大密度投影(MIP)、多平面重组(MPR)、曲面重组(CPR)]和DR图像分别由2位骨关节组放射医师实行非双盲阅片,阅片结果分别与手术后病理或穿刺病理学结果对比分析。统计学处理采用χ~2检验。结果 86例患者中,MSCT检出85例,DR检出78例,检出率差异有统计学意义(χ~2=5.74,P<0.05);MSCT定性诊断正确82例,DR定性诊断正确71例,定性诊断正确率差异有统计学意义(χ~2=7.16,P<0.01)。结论在四肢骨关节结核的检出及定性诊断方面,MSCT较DR均具有明显优势。MSCT在四肢骨关节结核的临床诊断中有重要应用价值。更多还原

【Abstract】 Objective To evaluate the clinical value and the influencing factors of multi-slice CT(MSCT)and directly digital radiography device(DR)in the diagnosis of bone and joint tuberculosis(BJTB). Methods Eighty-six patients with bone and joint tuberculosis proved by surgical pathology or biopsy pathology were examined by MSCT and DR before clinical diagnosis. All the images of MSCT(including CT scan,VRT,MIP,MPR,CPR) and DR were viewed by two groups of radiologists separately. The results of MSCT and DR were compared with surgical pathology or biopsy pathology. Statistics processing with chi-square test. Results Eighty-six patients with BJTB were checked out by surgical pathology or biopsy pathology of which 85 were checked out by MSCT,and 78 were by DR(χ~2=5.74,P<0.05); while 82 were diagnosed correctly on MSCT,and 71 were on DR(χ~2=7.16,P<0.01). The differences between MSCT and DR were statistically significantly. Conclusion MSCT is superior to DR in the diagnosis of BJTB,MSCT has an important application value in the diagnosis of BJTB.更多还原
