【摘要】 目的探究穿刺引流微创术联合术后尿激酶溶解治疗对高血压性脑出血的治疗效果、并发症以及预后的影响。方法选取2013年1月—2016年1月江苏省连云港市东海县人民医院神经外科收治的80例高血压性脑出血患者为研究对象。采用随机数字表法将患者分为观察组和对照组,各40例。对照组患者进行传统开颅术治疗,观察组患者进行穿刺引流微创术联合术后尿激酶溶解治疗。观察患者的治疗效果、并发症(尿路感染、肺部感染、颅内感染、消化系统出血、深静脉血栓)发生率和格拉斯哥预后量表(GOS)评分等级。结果观察组患者总有效率高于对照组(P<0.05)。观察组患者尿路感染、肺部感染、颅内感染、消化系统出血、深静脉血栓发生率低于对照组(P<0.05)。观察组患者GOS评分等级优于对照组(P<0.05)。结论穿刺引流微创术联合术后尿激酶溶解治疗对高血压性脑出血有良好的治疗效果,可降低并发症发生率,改善预后,值得临床推广。更多还原

【Abstract】 Objective To investigate the effect ofminimally invasive surgery combined with urokinase on the therapeutic effect, complications and prognosis of hypertensive intracerebral hemorrhage. Methods 80 patients with hypertensive intracerebral hemorrhage admitted in Department of Neurosurgery of Donghai County People’s Hospital from January2013 to January 2016 were selected as subjects. The patients were divided into observation group and control group by random number table method,40 cases in each group. Patients in control group were treated with traditional craniotomy,while patients in observation group were treated withminimally invasive surgery combined with urokinase. The therapeutic effect, complications( urinary tract infection,pulmonary infection,intracranial infection,digestive system hemorrhage,deep venous thrombosis) and the Glasgow Outcome Scale( GOS) score were observed. Results The total effective rate of observation group was higher than that of control group( P < 0. 05). The incidence of urinary tract infection, pulmonary infection, intracranial infection,gastrointestinal bleeding and deep venous thrombosis in observation group was lower than that in control group( P < 0. 05). The GOS score of observation group was better than that of control group( P < 0. 05). Conclusion Minimally invasive surgery combined with urokinase has a good therapeutic effect on hypertensive cerebral hemorrhage, can reduce the incidence of complications and improve the prognosis,which is worthy of clinical promotion.更多还原
