不同剂量阿托伐他汀钙治疗对冠心病患者外周血内皮祖细胞的影响
Effects on endothelial progenitor cells in different doses of atorvastatin calcium in the treatment of patients with coronary heart disease
【摘要】 目的探讨不同剂量阿托伐他汀钙治疗对冠心病患者外周血内皮祖细胞(EPC)的影响。方法选取自2013年1月至2015年1月收治的冠心病患者96例,用随机数表法分为观察组和对照组,每组48例。两组患者均给予阿托伐他汀治疗,对照组40 mg/d,观察组20 mg/d,疗程为8周。分别观察用药前(用药后0 h)及用药后6 h、18 h、36 h、60 h、108 h、150 h等时段EPC增殖、迁移及黏附能力。结果两组患者的EPC增殖情况均呈波浪状分布,在72 h时达到最大值,72 h之前对照组的增幅大于观察组(P<0.05),72 h之后两组增长程度差异无显著性(P>0.05);黏附情况比较,观察组在108 h时达到最大值,至150 h时维持较稳定状态,对照组72 h时达到最大值,之后下降明显,较长时间应用大剂量阿托伐他汀对EPC的黏附能力无较大提升,差异无显著性(P>0.05);两组患者的EPC迁移情况均呈波浪状分布,在72 h时达到最大值,72 h之前对照组的增幅大于观察组(P<0.05),72 h之后对照组下降明显,较长时间应用大剂量阿托伐他汀对EPC的迁移能力无较大提升,差异无显著性(P>0.05)。结论使用20 mg/d或40 mg/d剂量的阿托伐他汀钙药物治疗可对EPC均有一定增殖作用,但较长时间的检测表明,应用大剂量(40 mg/d)阿托伐他汀钙对EPC的增殖、黏附和迁移能力无较大提升。
【Abstract】 Objective To investigate the influence of endothelial progenitor cells( EPC) of atorvastatin calcium therapy on coronary heart disease. Methods 98 cases of CHD patients were selected from January 2013 to January 2015. A random number table was used to divide these patients. All these patients were divided into observation group and control group of 48 cases. Two groups of patients were treated with atorvastatin therapy,the control group 40 mg / d,the observation group 20 mg / d,for 8 weeks. They were observed before treatment( after treatment 0 h) and after treatment 6 h,18 h,36 h,60 h,108 h,150 h. These indexes included EPC proliferation,migration and adhesion ability. Results EPC proliferation of two groups of patients were wavy distribution,reaching a maximum at 72 h. The increase 72 h before in the observation group is greater than the control group( P < 0. 05),the degree of growth in the two groups did not have difference after 72 h( P > 0. 05). The adhesion in the group reaches the maximum value 108 h,maintain a relatively stable state to 150 h,while the control group reaches a maximum at 72 h and then decreased significantly longer time high dose of atorvastatin on EPC adhesion ability without comparing big upgrade( P > 0. 05). EPC migration in two groups of patients showed wavy distribution,reaches a maximum at 72 h,the increase in the control group 72 h before is greater than those of the observation group( P < 0. 05). The control group decreased significantly after 72 h,long- time application of high- dose atorvastatin on EPC had much improvement in migration capability( P > 0. 05). Conclusion The use of atorvastatin calcium therapy may have a proliferation of EPC,but there is no apparent dose relationship.
