脑脊液和血清肿瘤标志物β-hCG、αFP和PLAP对中枢神经系统生殖细胞肿瘤的诊断价值的比较

【摘要】 目的探讨中枢神经系统生殖细胞肿瘤(ICGGTs)患者脑脊液和血清β-人绒毛膜促性腺激素(β-h CG)、甲胎蛋白(αFP)、胎盘碱性磷酸脂酶(PLAP)表达变化及临床诊断价值。方法采用双抗体夹心法测定19例ICGGTs患者(病例组)、22例中枢神经系统朗格汉斯细胞增生症(LCP)患者(对照组)的脑脊液和血清β-h CG、αFP、PLAP水平,并对其进行统计学处理及ROC曲线分析。结果病例组脑脊液和血清β-h CG、αFP、PLAP水平均显著高于对照组,差异有统计学意义(均P<0.05);ICGGTs患者中脑脊液β-h CG水平高于血清β-h CG水平,而血清αFP水平高于脑脊液αFP水平(均P<0.05);脑脊液β-h CG曲线下面积为0.933,敏感度、特异度分别为72.4%和100.0%;血清αFP曲线下面积为0.931,敏感度、特异度分别为89.6%和100.0%。结论脑脊液和血清肿瘤标志物β-h CG、αFP和PLAP在ICGGTs与LCP鉴别诊断中具有较高的应用价值,其中脑脊液β-h CG和血清αFP的诊断价值最高。

【Abstract】 Objective To investigate the diagnostic value of tumor markers β- h CG,αFP and PLAP in cerebrospinal fluid and serum patients with central nervous system germ cell tumors. Methods Double antibody sandwich method was used to determine the cerebrospinal fluid and serum β- h CG,αFP and PLAP levels in 19 patients with ICGGTs( case group) and 22 cases of central nervous system( LCP) patients( control group). The analysis of the ROC curve and the statistical analysis were performed. Results The levels of CSF and serum β- h CG,FP and PLAP were significantly higher than those in the control group( P < 0. 05). The area under the β- h CG curve of cerebrospinal fluid was 0. 933.The sensitivity and specificity respectively were 72. 4% and 100%. The area under the αFP curve was 0. 931,the sensitivity and specificity respectively were 89. 6% and 100%. Conclusion Cerebrospinal fluid and serum tumor markers β- h CG,FP and PLAP have a high value in the differential diagnosis of ICGGTs and LCP,and the diagnostic value of β- h CG in αFP was the highest.
