【摘要】 目的探讨微血管减压术治疗三叉神经痛的临床疗效。方法选择我院2014年6月至2015年8月收治的211例三叉神经痛患者,用随机数字表法分为治疗组106例和对照组105例。2组均进行常规基础治疗,对照组另给予卡马西平、苯妥英钠等常规药物治疗,治疗组给予微血管减压术治疗。比较2组临床疗效、视觉模拟分析法(VAS)疼痛评分、不良反应发生情况及复发情况。结果治疗组总有效率96.2%,明显高于对照组的79.0%(Z=-2.875,P=0.004)。治疗后,治疗组VAS评分由治疗前(23.2±2.1)分显著下降至(9.9±1.6)分(P<0.01),而且明显低于对照组评分(15.6±1.4)分(P<0.01)。治疗后随访半年,治疗组发生并发症9例(8.5%),对照组并发症19例(18.1%)(χ2=4.228,P=0.040)。治疗组复发3例(2.8%),对照组复发17例(16.2%)(χ2=10.974,P=0.001)。结论微血管减压术治疗三叉神经痛临床疗效显著,VAS疼痛评分明显降低,不良反应发生率及复发率低,可以有效改善患者生活质量,值得推广应用。更多还原

【Abstract】 Objective To investigate the clinical efficacy of microvascular decompression operation(MVD)in the treatment of patients with trigeminal neuralgia(TN).Methods Two hundred and eleven patients,who were in our hospital from June 2014 to August 2015,were randomly divided into treatment group(n=106)and control group(n=105).All the patients were treated by the routine therapy.The control group was treated by carbamazepine and phenytoin and the treatment group was treated by microvascular decompression operation.We compared the clinical efficacy,visual analogue analysis(VAS)pain score,adverse reaction and recurrence of the two groups.Results The total effective rate of the treatment group was 96.2%,which was significantly higher than that of 79.0%in the control group(Z=-2.875,P=0.004).After treatment,the VAS score of the treatment group was significantly decreased from(23.2±2.1)to(9.9±1.6)points(P<0.01),and was also significantly lower than that of(15.6±1.4)points in the control group(P<0.01).At the 6th month follow-up after treatment,9cases(8.5%)had complications in the treatment group,which was significantly lower than that of19cases(18.1%)in the control group(χ2=4.228,P=0.040).Three cases(2.8%)had recurrence in the treatment group,which was significantly lower than that of 17cases(16.2%)in the control group(χ2=10.974,P=0.001).Conclusion The clinical efficacy of MVD was significant in the treatment of TN.The VAS pain score is significantly reduced and it has lower incidence of adverse reactions and recurrence rate,which effectively improve the quality of life of patients.更多还原
