2型糖尿病患者尿微量白蛋白排泄率与eGFR的关系及相关危险因素

【摘要】 目的:分析2型糖尿病(T2DM)患者尿微量白蛋白排泄率(UAER)与肾小球滤过率(eGFR)的关系,并探讨相关的危险因素。方法:选取我院收治的2型糖尿病患者470例,根据UAER及eGFR的检查结果进行分组,收集患者的一般资料,研究UAER与e GFR的关系,并分析其相关危险因素。结果:不同UAER体质量、体表面积、BMI,DBP,SBP,高血压病程、TG,TC,血肌酐以及尿酸水平比较,差异有统计学意义(P<0.05)。不同e GFR年龄、体质量、糖尿病病程、体表面积、BMI、高血压的病程、HbA1c,HDL-C,血肌酐以及尿酸水平比较,差异有统计学意义(P<0.05)。大量清蛋白尿患者肾功能异常发生率最高,正常清蛋白尿患者肾功能异常发生率最低,差异有统计学意义(P<0.05)。eGFR≥90 mL·min-1·1.73 m-2患者正常、微量、大量清蛋白尿分别占82.08%、13.68%、4.25%,根据交叉检验分析,UAER与e GFR间有相关性(P<0.05)。Logistic回归分析结果显示,与UAER相关危险因素包括血肌酐、收缩压及体质指数,与e GFR相关危险因素包括年龄、体质量、血肌酐(P<0.05)。结论:UAER与e GFR间存在相关性,一并进行检测对早期诊断2型糖尿病肾损伤有利。而两者相关危险因素包括血肌酐、收缩压及体质指数。

【Abstract】 Objective:To analyze the relationship between urinary albumin excretion rate(UAER) and glomerular filtration rate(e GFR) in patients with type 2 diabetes mellitus(T2DM) and explore the related risk factors.Methods:470 patients with type 2 diabetes mellitus who were treated in our hospital were selected on the basis of the UAER and e GFR.Then the general information of patients was collected,and the relationship between UAER and e GFR and related risk factors were analyzed.Results:There was statistically significant difference about the body weight,body surface area,BMI,DBP,SBP,TG,TC,hypertension,serum creatinine and uric acid of patients with different UAER(P<0.05).There was statistically significant difference about the age,body mass,diabetes duration,body surface area,BMI,hypertension,HbA1 c,HDL-C,serum creatinine and uric acid levels of patients with different e GFR(P<0.05).The incidence rate of abnormal renal function was highest in patients with large quantity of albuminuria and lowest in patients with normal albuminuria,and the difference was statistically significant(P<0.05).Patients of e GFR ≥90ml·min-1·1.73m-2 with normal,small amount of and large quantity of albuminuria accounted for 82.08%,13.68% and 4.25%,respectively.According to the cross validation analysis,UAER and EGFR were correlated(P<0.05).Logistic regression analysis showed that the risk factors of UAER included serum creatinine,systolic blood pressure and BMI,and the risk factors of e GFR were age,body mass and serum creatinine(P<0.05).Conclusion:There is correlation between UAER and e GFR,and the combined detection should be conducive to early diagnosis of type 2 diabetic renal injury.The related risk factors include serum creatinine,systolic blood pressure and BMI.
