【摘要】 目的:研究应用椎弓根螺钉置入内固定系统治疗胸腰段骨折脱位患者的临床效果。方法:回顾分析我院52例胸腰段骨折脱位患者临床资料,应用椎弓根螺钉置入内固定系统治疗,对比实施内固定前后及随访1年后脊柱损伤、骨折复位的恢复情况,Cobb角、前缘高度百分比、后缘高度百分比、矢状位指数、楔变指数、VAS评分。结果:各时期A级比例,无统计学意义(P>0.05)。B-E级中,治疗后、随访1年与治疗前相较,差异具有显著性(P<0.05),随访1年与治疗后相较,无统计学意义(P>0.05)。后缘高度在治疗前后、随访1年中对比,无统计学意义(P>0.05)。前缘高度、Cobb角在治疗后、随访1年中与治疗前相较,差异具有显著性(P<0.05),前缘高度、Cobb角在治疗后、随访1年中对比,无统计学意义(P>0.05)。治疗后、随访1年矢状位指数、楔变指数、VAS评分与治疗前相较,呈显著差异(P<0.01),但随访1年与治疗后相较,无统计学意义(P>0.05)。结论:胸腰段骨折脱位患者应用椎弓根螺钉置入内固定系统治疗效果显著,应予推广。更多还原

【Abstract】 Objective:To study the clinical effect of pedicle screw fixation system in the treatment of patients with thoracolumbar fracture and dislocation.Methods:We retrospectively analyzed the clinical data of 52 cases of thoracolumbar fracture and dislocation patients.They were given pedicle screw fixation system in the treatment.We compared such aspects before and after the fixation and 1-year follow up as recovery of spinal injury and fracture reduction,Cobb angle,the percentages of anterior height and posterior height,sagittal index,wedge index and VAS score.Results:The proportion of class A at each period had no statistical difference(P>0.05).Class B-E had significant difference at after treatment and 1-year follow-up as compared with before treatment(P<0.05),but no statistical difference between 1-year follow-up and right after treatment(P>0.05).Compared with before treatment,the posterior height had no statistical difference after treatment and in the 1-year follow-up(P>0.05),however,the anterior height and Cobb angle had significant difference after treatment and in the 1-year follow-up(P<0.05).Yet the anterior height and Cobb angle after treatment showed no statistical significance as compared to 1-year follow-up(P>0.05).In addition,the sagittal index,wedge index and VAS score showed significant difference after treatment and 1-year follow-up as compared with before treatment(P<0.01),but no statistical difference between after treatment and 1-year follow-up(P>0.05).Conclusion:The pedicle screws internal fixation system in the treatment of patients with dislocation of thoracolumbar fracture was conducive not only to the reduction of fracture,but also to the recovery of spinal cord function.So it had significant effect and should be popularized.更多还原
