【摘要】 目的:探讨纤维支气管镜在急诊鼻胃管置入患者中的应用效果。方法:将行鼻胃管置入者,以收治顺位分为单数(A组,纤维支气管镜引导下置入鼻胃管)和双数(B组,传统鼻胃管插管法)组。比较两组鼻胃管置入后1、3、5、15 min血流动力学变化,一次性置管成功率、鼻胃管置入时间及并发症。结果:与置入前比,两组鼻胃管置入1 min后收缩压(SBP)明显升高,血氧饱和度(Sp O2)明显降低,呼吸频率(RF)、HR明显加快(P<0.05)。A组鼻胃管置入后1 min SBP升高幅度、Sp O2降低幅度、RP和HR增加次数均低于B组(P<0.05)。A组一次置管成功率(100.0%)高于B组(80.7%)(χ2=6.016,P<0.05);A组鼻胃管平均置入时间((5.3±1.7)min)低于B组((9.1±2.8)min)(t=4.217,P<0.05);A组并发症发生率(3.1%)低于B组(22.6%)(χ2=6.317,P<0.05)。结论:纤维支气管镜引导下鼻胃管一次置入成功率高,对患者血流动力学影响小,气道损伤轻。更多还原

【Abstract】 Objective:To investigate the applicaion effect of fiberoptic bronchoscopy in patients with nasogastric tube intubation. Methods:The patients with nasogastric tube intubation in sequence were divided into singular(Group A,nasogastric tube intubation by fiberoptic bronchoscopy)and dual(Group B,traditional nasogastric tube intubation)groups. The hemodynamic changes at 1,3,5,15 minutes after nasogastric tube intubation,one-time success rate of intubation,nasogastric tube intubation time and complications between two groups were compared. Results:Intubation than before,the systolic blood pressure(SBP)at one minute after nasogastric tube intubation between two groups significantly increased,the blood oxygen saturation(Sp O2)significantly decreased, the respiratory frequency(RF)and heart rate(HR)obviously quickened(P<0.05). Increased SBP,decreased Sp O2,increase times of RF and HR at one minute after nasogastric tube intubation in the Group A were lower than those group B(P<0.05). One-time success rate of intubation in the Group A(100.0%)was higher than that Group B(80.7%)(χ2=6.016,P<0.05). The mean time of nasogastric tube intubation in the Group A((5.3±1.7)minutes)was lower than that Group B((9.1±2.8)minutes)(t=4.217,P<0.05). The complications in the Group A(3.1%)was lower than that Group B(22.6%)(χ2=6.317,P<0.05).Conclusion:One-time success rate of nasogastric tube intubation by fiberoptic bronchoscopy is high,little influence of hemodynamics on patients and light airway damage.更多还原
