【摘要】 目的探讨基质金属蛋白酶3(MMP-3)mRNA在左半结肠癌(LSCC)、右半结肠癌(RSCC)组织中的表达及其与患者临床病理参数和预后的关系。方法选择结肠腺癌患者80例,以结肠脾曲为界分为LSCC、RSCC各40例。取手术切除的癌组织及其配对的癌旁正常结肠组织,采用Real-time qRT-PCR法检测不同结肠组织中MMP-3mRNA表达,并分析其表达与患者临床病理参数及预后的关系。结果 LSCC及RSCC组织中MMP-3 mRNA表达均高于其配对的癌旁正常结肠组织,且LSCC组织中MMP-3 mRNA表达明显高于RSCC组织(P<0.01)。无论LSCC还是RSCC,MMP-3 mRNA高表达均与患者脉管癌栓、异时性远处脏器转移有关(P均<0.05);MMP-3 mRNA高表达与LSCC患者肿瘤浸润深度及癌症相关死亡有关(P均<0.05)。Kaplan Meier生存分析显示,MMP-3 mRNA高表达者中位生存时间明显低于低表达者(P<0.01);分层分析发现,LSCC患者MMP-3 mRNA高表达者中位生存时间明显低于低表达者(P<0.05),MMP-3 mRNA表达对RSCC患者预后无明显影响(P>0.05)。结论 LSCC组织中MMP-3 mRNA表达明显高于RSCC组织;MMP-3 mRNA高表达仅是LSCC患者的不良预后因子。更多还原

【Abstract】 Objective To investigate the expression of matrix metalloproteinase 3( MMP-3) mRNA in the left-sided and right-sided colon cancer and its relationships with clinicopathological parameters and prognosis. Methods Eighty patients with colonic adenocarcinoma were divided into the left-sided colon cancer( LSCC) group and right-sided colon cancer( RSCC) group with forty patients in each by the boundary of splenic flexure. Eighty paired specimens were collected from each patient’s cancer tissues and itself normal colon mucosal tissues. The mRNA expression of MMP-3 was detected in two subgroups by real-time quantitative RT-PCR. Subsequently,the relationships between the expression and clinicopathological parameters as well as prognosis of each subgroup were analyzed. Results The mRNA expression of MMP-3 in the colon cancer tissues was significantly higher than that in the normal colon mucosa,and the MMP-3 mRNA expression in LSCC was higher than that in RSCC( P < 0. 01). In the Pearson χ2test,the high expression of MMP-3 was closely related with lymphatic / vascular invasion and metachronous distant metastasis for both RSCC and LSCC( all P < 0. 05). The high expression of MMP-3 mRNA was also closely related with depth of invasion and cancer-related death of LSCC patients( all P< 0. 05). Kaplan-Meier analysis showed that the high MMP-3 mRNA expression group had the poorer median survival time than that of the low expression group( P < 0. 01),and the stratification analysis showed the median survival time of RSCC patients with high expression of MMP-3 was less than that of low expression one( P < 0. 05). The expression of MMP-3mRNA had no effect on the prognosis of patients( P > 0. 05). Conclusion The expression of MMP-3 mRNA in the LSCC tissues is significantly higher than that in RSCC tissues,and the high expression of MMP3 is only a poor prognostic factor for LSCC patients.更多还原
