【摘要】 目的研究标准剂量的氯吡格雷对正常体重指数范围内不同体重的急性缺血性脑卒中患者的防治效果。方法入组2012年9月至2013年5月,在南京市第一医院神经内科住院的正常体重指数范围内诊断为AIS的患者126例,将其分性别取体重中位数等量分为高、低体重组各63例,给予标准剂量氯吡格雷抗血小板治疗,检测服药前及服药7 d后的血小板聚集率,随访患者1年内卒中再发率及不良事件发生率。结果两组患者服用氯吡格雷前血小板聚集率为低体重组74.1%±13.2%,高体重组74.7%±13.6%,P=0.802,无统计学差异,服药7d后低体重组34.4%±9.8%,高体重组39.1%±14.4%,P=0.034,有显著的统计学差异。随访1年,两组卒中再发率及不良事件发生率无统计学差异。结论正常体重指数范围内,较低体重的患者在服用氯吡格雷7 d后有更好的抗血小板作用。但此作用是否会减少缺血性卒中再发率或增加出血等不良事件发生率,仍需进一步研究。更多还原

【Abstract】 Objective Based on normal body mass index,to study the therapeutic effect of clopidogrel on different weight patients with acute ischemic stroke. Methods A total of 126 normal- weight patients with acute ischemic stroke in department of neurology,Nanjing First Hospital from September 2012 to May 2013 were enrolled and evenly divided by gender and median weight into low and high weight groups. Platelet aggregation was detected before and 7 days later after taking standard clopidogrel antiplatelet therapy,and stroke recurrence rate and adverse event occurrence rate was followed- up for one year. Results No significant difference in platelet aggregation before( low weight group was 74. 1% ± 13. 2% and high weight group was 74. 7% ± 13. 6%,P = 0. 802) while statistical difference 34. 4% ± 9. 8% and 39. 1% ± 14. 4% respectively,P = 0. 034) was found 7 days later after taking standard clopidogrel antiplatelet therapy. Two groups of stroke recurrence rate and adverse event occurrence rate showed no significant difference in one year’ s following up. Conclusion Within the range of normal BMI,patients with a lower body weight would have a better anti- platelet effect after taking clopidogrel. But further research was needed to see whether such effect would reduce stroke recurrence or increase adverse events.更多还原
