【摘要】 目的分析高脂血对临床生化测定的影响,探讨利用α-环糊精(α-CD)消除高脂血对常用临床生化指标测定的影响。方法使用α-环糊精处理正常对照组、模拟脂血组、患者脂血组血清,使用OLYMPUS AU-640全自动生化分析仪分别测定处理前后临床常用生化指标,将测定结果进行统计学分析。临床可接受性能判断以美国临床实验室修正案(CLIA’88)允许总误差(TE a)的1/2为标准,判断其偏差是否为临床可接受。结果 1正常对照组与模拟脂血组比较,除ALP、GGT、LDH、GLU、CK、α-AMY的平均偏差低于1/2TE a外,其余常用临床生化指标的平均偏差均高于1/2 TE a;2正常对照组、模拟脂血组使用α-CD处理前后,除TP、α-AMY、CHO、TG的平均偏差高于1/2TE a外,其余常用临床生化指标的平均偏差均低于1/2 TE a;3患者脂血组血清经α-CD处理后,其常用临床生化指标测定结果明显得到改善,其测定值更接近真实值。结论高脂血影响大部分临床常用生化指标测定的准确性,使用α-环糊精可以消除高脂血对大部分临床常用生化检测指标测定结果的影响。更多还原

【Abstract】 Objective To analyze the effect of high fat blood on clinical biochemical measurement,and to explore the effect of the alpha-cyclodextrin(-CD) on the determination of common clinical biochemical indexes.Methods The serum of normal control group,simulated lipid group,patient blood lipid group was processed by Alpha cyclodextrin. AU-640 OLYMPUS automatic biochemical analyzer was used before and after the treatment to determine the clinical biochemical indicators,the results were statistically analyzed.Clinical acceptable judgment in the United States clinical laboratory amendment( CLIA’88) allows the total error of 1 / 2 as standard( TE a) to determine whether the deviation is clinically acceptable.Results 1In the comparison of normal control group and simulated lipid group,the average deviation of ALP、GGT、LDH、GLU、CK、α-AMY were lower than 1 / 2TE a,but the other commonly used clinical biochemical indicators are higher than the 1 / 2 TE a.2In the normal control group and simulated lipid group,the average deviation of TP、α-AMY、CHO、TG were higher than that of 1 / 2TE a before and after treatment,but the average deviation of the other commonly used clinical biochemical indexes were lower than that of the 1 / 2 TE a.3The common clinical biochemical index determination results of serum in the patient group after treatment of alpha CD had been improved obviously.The measured values were close to the true value.Conclusion High blood fat has effect on the accuracy of most commonly used biochemical indexes in clinic,the use of alpha cyclodextrin can eliminate the effect of high fat blood on the determination results of most commonly used biochemical tests.更多还原
