精液优化处理前后精子DNA碎片率和畸形率的变化及对IVF妊娠结局的影响
The influence of semen optimization treatment on changes of sperm DNA fragmentation rate and abnormal rate and the impact on IVF pregnancy outcome
【摘要】 目的比较精液优化处理前后精子DNA碎片率和畸形率的变化及对IVF妊娠结局的影响。方法采用精子染色质扩散(SCD)方法检测计算精子DNA碎片指数(DFI),采用快速染色(DiffQuik)法测定精子畸形率,分析探讨精液优化处理前后精子DNA碎片率与畸形率的变化及与临床妊娠率的关系。结果处理后精子DNA碎片率及畸形率显著低于处理前(P<0.05),精液优化处理后畸精组的临床妊娠率与正常对照组无显著性差异(P>0.05),DFI<30%组的临床妊娠率显著高于DFI≧30%组(P<0.05)。结论密度梯度离心结合上游法处理精液后可以显著降低精子DNA碎片率及畸形率,精子DNA碎片率可以用来预测IVF的妊娠结局。
【Abstract】 Objective Compare the influence of semen optimization on changes of sperm DNA fragmentation rate and abnormal rate and the impact on IVF pregnancy outcome.Methods Using sperm chromatin dispersion( SCD) method to detect sperm DNA fragmentation index calculation( DFI),using rapid staining( Diff- Quik) determination to determine the rate of teratosperm.Analyzed and discussed the change of DNA fragmentation rate and deformity rate of sperm before and after the optimization process,and their relationship with clinical pregnancy rate.Results Sperm DNA fragmentation rate and malformation rate significantly decreased after the treatment( P < 0.05).The pregnancy rate of sperm optimization group control group was not significantly different from each other( P > 0.05).Clinical pregnancy rate of low DFI group( DFI< 30%) was significantly higher than that in high DFI group( DFI ≧ 30%,P < 0.05).Conclusions After the density gradient centrifugation combined with upstream treatment can significantly reduce sperm DNA fragmentation rate and abnormal rate of sperm.Sperm DNA fragmentation rate can be used to predict IVF pregnancy outcomes.
