【摘要】 目的探讨甲泼尼龙(MP)联合鼠神经营养因子(m NGF)治疗椎间孔镜下髓核摘除术后神经根刺激症状的疗效。方法回顾性选取2010年6月—2013年6月河北省人民医院符合纳入标准的单间隙腰椎间盘突出症患者43例为研究对象。患者行椎间孔镜下髓核摘除术,术中以MP(80 mg)和mNGF(30μg)浸润的明胶海绵覆盖神经根,术后给予常规甘露醇125 ml静脉滴注,2次/d,m NGF肌肉注射,1次/d,共3 d。术前、术后即刻、术后1 d、术后3 d及末次随访时采用视觉模拟评分法(VAS)评估患者术后腰腿痛程度。结果术后即刻VAS评分高于术前(P<0.05);术后1 d、术后3 d、末次随访时VAS评分低于术前、术后即刻(P<0.05);术后3 d、末次随访时VAS评分低于术后1 d(P<0.05)。结论 MP联合m NGF可有效改善椎间孔镜下髓核摘除术后神经根刺激症状。更多还原

【Abstract】 Objective To investigate the curative effects of methylprednisolone( MP) combined with mouse nerve growth factor( m NGF) in the nerve roots irritation after percutaneous transforaminal endoscopic discectomy( PTED). Methods43 patients with single gap lumbar disc herniation who had met the inclusion standard from June 2010 to June 2013 in Hebei General Hospital were selected retrospectively in this study. Patients were treated with PTED,during which put the infiltrative gelatin sponge of MP( 80 mg) and m NGF( 30 μg) cover nerve roots,intravenous drip of 125 ml routine mannitol 2 times / d postoperatively,m NGF muscle injection 1 time / d,and totally 3 days. Visual analogue scale( VAS) was used pre- operatively,immediately after operation,1 day after operation,3 days after operation and at the time of last follow- up to assess lumbocrural pain degree of patients postoperatively. Results VAS score immediately after operation was higher than that of pre- operation( P< 0. 05); VAS score 1 day after operation,3 days after operation and at the time of last follow- up was lower than that of pre-operation and immediately after operation( P < 0. 05); VAS score 3 days after operation and at the time of last follow- up was lower than that of 1 day after operation( P < 0. 05).
