【摘要】 随着经济、社会的快速发展,慢性病患病率、病死率逐年升高。本文参阅了国内外有关慢性病管理的最新研究文献,对慢性病管理的概念、意义、对象及国内外慢性病管理研究进展进行了综述,同时对我国未来慢性病管理模式进行了展望,提出:以社区为依托,综合性三级甲等医院全科医疗科联合各专科及基层医院对慢性病进行持续、综合性的管理,有利于提高社区全科医生规范化管理慢性病的水平,有利于提高慢性病的知晓率和控制率。更多还原

【Abstract】 With the rapid development of economy and society,the prevalence and mortality rate of chronic disease are increasing year by year.Based on some latest literatures about chronic disease management at home and abroad are consulted,the concept,the meaning and the object of chronic disease management,and the advances of chronic disease management at home and abroad are reviewed.Meanwhile,the chronic disease management model in our country in the future is prospected.With the support of community,continuous and comprehensive chronic disease management performed by a combination of department of general practice in class 1 & grade 3 general hospitals,special hospitals and primary hospitals,can improve the level of standardized management of community general practitioners in the field of chronic disease management,and can also improve the awareness and the control rate of chronic disease.更多还原
