热性惊厥患儿神经元特异性烯醇化酶血清水平及脑电图分析
Analysis of serum levels of neuron specific enolase and electroencephalographic in children with febrile seizures
【摘要】 目的探讨神经元特异性烯醇化酶(NSE)血清水平和脑电图(EEG)在热性惊厥(FS)患儿中的改变,揭示NSE血清水平和EEG改变与FS患儿脑损伤中的关系。方法选择2011年1月至2014年12月在我院确诊的热性惊厥(FS)患儿184例,其中单纯性热性惊厥(SFS组)152例、复杂性热性惊厥(CFS组)32例,另选40例健康儿童(健康对照组)作为对照组。应用Elecsys2010全自动免疫分析仪电化学发光法分别测定3组血清中NSE水平,对热性惊厥患儿于病情稳定第1天和第14天进行EEG检查,并进行分析。结果 NSE血清水平在CFS和SFS患儿中均高于健康对照组(P<0.01);CFS患儿血清NSE水平高于SFS组(P<0.01)。惊厥发作后第1天152例SFS患儿64例EEG异常(42.1%),32例CFS患儿17例异常(52.1%),两组比较差异无统计学意义(P>0.05);第14天SFS患儿仅27例异常(17.8%),而CFS组15例异常(46.9%),两组比较差异具有统计学意义(P<0.01);惊厥发作后第1天EEG异常的FS患儿NSE血清水平明显高于脑电图正常者,二者比较差异有统计学意义(P<0.01)。结论 NSE血清水平可作为FS惊厥患儿神经元细胞损伤的特异性指标,结合EEG检查判断脑损伤程度,对指导治疗及评估预后有重要的临床价值。
【Abstract】 Objective To investigate serum levels of neuron-specific enolase and electroencephalographic in children with febrile seizures and to indicate the relationship between serum levels of neuron-specific enolase and abnormal electroe-ncephalographic and brain injury in children with febrile seizures.Methods Serum levels of neuron-specific enolase in 152 children with simple febrile seizures,32 children with complex febrile seizures and 40 healthy children were measured by electrochemilumines,and electroencephalogram in 1day and 14 days after the seiture,and analyzed.Results Serum levels of neuron-specific enolase in complex febrile seizures group and simple febrile seizures group were higher than those in control group(P<0.01).Serum levels of neuron-specific enolase in complex febrile seizures group were higher than those in control group(P<0.01).Of the 152 patients with simple febrile seizures,64(42.1%)abnormal on electrochemilumines,32 patients with complex febrile seizures,17(52.1%)abnormal on electrochemilumines in 1day after the seizures,there was no statistical significance between them(P>0.05).And in 14 days after the seizures,27(17.8%)abnormal on electrochemilumines in children with simple febrile seizures,15(46.9%)abnormal on electrochemilumines in children with complex febrile seizure,there was statistical significance between them(P<0.01).And serum levels of neuron-specific enolase in abnormal on electrochemi-lumines in children with all febrile seizures were higher than those in normal on electrochemilumines,there was statistical significance between them(P<0.01).Conclusion The serum levels of neuron-specific enolase can be regarded specific indicator of brain injury in children with febrile seizures,and combined electrochemilumines examination can evaluate brain injury after the febrile seizures.
